中医理论将冠心病(CHD)合并抑郁状态归属于"胸痹心痛"合并"郁证"的范畴,认为该病的核心病机为"虚气留滞",即脾气亏虚为始动病机、气痰瘀滞为直接病机.二者互为因果,导致心脉痹阻与心神失养并存,呈现痹证与郁证交织的现象.基于王永炎院士"虚气留滞"理论,本团队提出"补虚行滞、痹郁同调"的治疗思路,主张以健脾益气治其本、行气化痰活血治其标,方用养心通络膏加减,从而实现心脾兼顾、气血并治、痹郁同调.

In Chinese medicine theory,coronary heart disease(CHD)complicated with depressive state falls into the categories of"chest bi-impediment/cardiac pain"and"depressive disorder."It is believed that the core pathogenesis is"qi deficiency leading to pathogenic accumulation."That is,spleen qi deficiency is the initiating pathogenesis,while stagnation of qi,phlegm,and blood stasis constitutes the direct pathogenesis.These two aspects are mutually causative,leading to the coexistence of heart vessel obstruction and malnourishment of heart spirit,manifesting as an intertwined state of bi-impediment disorder and depressive disorder.Based on Academician WANG Yongyan's theory of"qi deficiency leading to pathogenic accumulation",our research group proposes a therapeutic approach of"supplementing deficiency and resolving accumulation,and simultaneously regulating bi-impediment disorder and depressive disorder."This principle advocates strengthening the spleen and supplementing qi to treat the root,and circulating qi,transforming phlegm,and circulating blood to address the manifestations.The modified Yangxin Tongluo Paste is used to achieve a comprehensive treatment that takes into account both the heart and spleen,regulates both qi and blood,and simultaneously addresses bi-impediment disorder and depressive disorder.
