超声弹性成像联合超声引导下细针穿刺活检对甲状腺结节良恶性的鉴别诊断价值OA
Differential Diagnostic Value of Ultrasound Elastography Combined with Ul-trasound-guided Fine Needle Aspiration Biopsy for Benign and Malignant Thyroid Nodules
目的 探讨超声弹性成像(UE)联合超声引导下细针穿刺活检(US-FNAB)在甲状腺结节良恶性鉴别诊断中的应用价值.方法 回顾性收集 2022 年 10 月至 2025 年 10 月联勤保障部队第九〇七医院收治的 80 例甲状腺结节患者的临床资料,患者共检出 92 个甲状腺结节,所有结节均经手术病理检查证实性质,术前均接受UE检查和US-FNAB检查.以手术病理检查结果作为诊断"金标准",分析UE联合US-FNAB对甲状腺结节良恶性的鉴别诊断效能;并比较甲状腺良恶性结节在UE参数和US-FNAB Bethesda分级分布方面的差异.结果 病理检查结果显示,92 个甲状腺结节中恶性结节 41 个(44.57%),良性结节 51 个(55.43%);在鉴别甲状腺结节良恶性方面,UE联合US-FNAB的诊断效能最高(P<0.05);在UE参数方面,恶性结节的弹性评分、应变率比值、蓝色区域占比、弹性指数明显高于良性结节(P<0.05);在US-FNAB Bethesda分级分布方面,良性结节主要分布于BethesdaⅡ级,而恶性结节则主要集中于BethesdaⅤ~Ⅵ级,良性结节与恶性结节的Bethesda分级分布差异显著(P<0.05).结论 UE与US-FNAB联合应用能显著提高对甲状腺结节良恶性的鉴别诊断效能,且两者参数联合可为识别甲状腺良恶性结节提供重要参考.
Objective To explore the application value of ultrasound elastography(UE)combined with ultrasound-guided fine needle aspiration biopsy(US-FNAB)in the differential diagnosis of benign and malignant thyroid nodules.Methods Clinical data of 80 patients with thyroid nodules admitted to the 907th Hospital of the Joint Logistics Support Force from October 2022 to October 2025 were retrospectively collected.A total of 92 thyroid nodules were detected in these patients,all of which were confirmed by surgical pathology.All patients underwent UE examination and US-FNAB examination preoperatively.Taking surgical pathological results as the diagnostic"gold standard",the diagnostic efficacy of UE combined with US-FNAB in differentiating benign and malignant thyroid nodules was analyzed.The differences in UE parameters and US-FNAB Bethesda grading distribution between benign and malignant thyroid nodules were compared.Results Pathological results showed that among the 92 thyroid nodules,41(44.57%)were malignant and 51(55.43%)were benign.UE combined with US-FNAB showed the highest diagnostic efficacy in differentiating benign and malignant thyroid nodules(P<0.05).In terms of UE parameters,elasticity score,strain ratio,proportion of blue area,and elasticity index were significantly higher in malignant nodules than those in benign nodules(P<0.05).Regarding US-FNAB Bethesda grading distribution,benign nodules were mainly distributed in Bethesda grade Ⅱ,while malignant nodules were mainly concentrated in Bethesda grades Ⅴ~Ⅵ.The distribution of Bethesda classifications between benign and malignant nodules showed significant difference(P<0.05).Conclusions The combined application of UE and US-FNAB can significantly improve the differential diagnosis efficiency of benign and malignant thyroid nodules,and the combined parameters of both methods provide important references for identifying benign and malignant thyroid nodules.
