Efficacy Comparison of Neuroendoscopic Transsphenoidal Resection and Mi-croscopic Transsphenoidal Resection in the Treatment of Sellar Region Tumors

目的 对比分析神经内镜下经鼻蝶切除术与显微镜下经鼻蝶切除术治疗鞍区肿瘤的临床效果.方法 选取 2021 年 1月至 2024 年 9 月我院收治的鞍区肿瘤患者 77 例,根据手术方案不同分为神经内镜组(n=40,采用神经内镜下经鼻蝶切除术治疗)和显微镜组(n=37,采用显微镜下经鼻蝶切除术治疗).对比两组的肿瘤全切率、围手术期指标、视觉功能[视野指数(VFI)、模式标准差(PSD)]、激素[生长激素(GH)、促肾上腺皮质激素(ACTH)、催乳素(PRL)]水平及并发症发生率.结果 神经内镜组的肿瘤全切率为 92.50%(37/40),明显较显微镜组的 75.68%(28/37)高(P<0.05);神经内镜组的住院时间明显较显微镜组短,术中失血量明显较显微镜组少(P<0.05);术后 1 个月,神经内镜组的VFI明显较显微镜组高,PSD明显较显微镜组低(P<0.05);术后 7 d,神经内镜组的血清GH、ACTH、PRL水平均明显较显微镜组低(P<0.05);神经内镜组的并发症总发生率为 2.50%(1/40),明显较显微镜组的 21.62%(8/37)低(P<0.05).结论 相较于显微镜下经鼻蝶切除术,神经内镜下经鼻蝶切除术用于治疗鞍区肿瘤,可明显提高肿瘤全切率,减少术中出血,促进患者视觉功能恢复,降低患者激素异常分泌及并发症发生风险,加快患者康复进程.

Objective To compare and analyze the clinical efficacy of neuroendoscopic transsphenoidal resection and microscopic transsphenoidal resection in the treatment of sellar region tumors.Methods A total of 77 patients with sellar region tumors admitted to our hospital from January 2021 to September 2024 were selected and divided into the neuroendoscopic group(n=40,treated with neuroendoscopic transsphenoidal resection)and microscopic group(n=37,treated with microscopic transsphenoidal resection)according to different surgical procedures.The total tumor resection rate,perioperative indicators,visual function[visual field index(VFI),pattern standard deviation(PSD)],hormone[growth hormone(GH),adrenocorticotropic hormone(ACTH),prolactin(PRL)]levels and complication rate were compared between the two groups.Results The total tumor resection rate in the neuroendoscopic group was 92.50%(37/40),significantly higher than 75.68%(28/37)in the microscopic group(P<0.05).The length of hospital stay was significantly shorter and intraoperative blood loss was significantly less in the neuroendoscopic group than those in the microscopic group(P<0.05).At 1 month postoperatively,the neuroendoscopic group showed significantly higher VFI and lower PSD than the microscopic group(P<0.05).At 7 days postoperatively,the serum levels of GH,ACTH,and PRL in the neuroendoscopic group were significantly lower than those in the microscopic group(P<0.05).The total complication rate in the neuroendoscopic group was 2.50%(1/40),significantly lower than 21.62%(8/37)in the microscopic group(P<0.05).Conclusions Compared with microscopic transsphenoidal resection,neuroendoscopic transsphenoidal resection applied in the treatment of sellar region tumors can significantly improve the total tumor resection rate,reduce intraoperative blood loss,promote the recovery of visual function,lower the risk of abnormal hormone secretion and complications,and accelerate the rehabilitation process of patients.
