Mechanism of Maimendong Decoction in the treatment of idiopathic pulmonary fibrosis in rats based on epithelial-mesenchymal transition and angiogenesis

目的 探讨麦门冬汤对特发性肺纤维化的干预作用及可能机制.方法 将 60 只Wistar 大鼠随机分为6 组,每组10 只.除正常组外,其余5 组大鼠采用气管内注射博莱霉素方法复制特发性肺纤维化模型.造模成功后,麦门冬汤低、中、高剂量组分别给予7.5 g/kg、15 g/kg、30 g/kg麦门冬汤灌胃,醋酸泼尼松组给予6.25 mg/kg 醋酸泼尼松灌胃,正常组及模型组给予等量生理盐水灌胃,均1 次/d,连续灌胃3 周.检测各组大鼠肺功能[用力肺活量(FVC)、用力最大呼气流速(PEF)、肺总阻力(RL)、动态顺应性(Cydn)],HE 染色及 Masson 染色观察肺组织病理形态,免疫组化法检测肺组织中 E-钙黏蛋白(E-cad)、α-平滑肌肌动蛋白(α-SMA)、Ⅰ型胶原蛋白(Col-lagen Ⅰ)阳性表达情况及微血管密度(CD34 标记),实时荧光定量PCR 法检测肺组织中 E-cad、α-SMA、Collagen Ⅰ、血管内皮生长因子(VEGF)、成纤维生长因子 2(FGF2)mRNA 表达情况.结果 与正常组比较,模型组大鼠 FVC、PEF、Cydn 均明显降低(P 均<0.05),RL、肺泡炎评分、肺纤维化评分均明显升高(P 均<0.05);肺组织中 E-cad 阳性表达平均光密度值及 E-cad mRNA 相对表达量均明显降低(P 均<0.05),α-SMA 和 CollagenⅠ阳性表达平均光密度值、微血管密度及 α-SMA、Collagen Ⅰ、VEGF、FGF2 mRNA 相对表达量均明显升高(P 均<0.05).与模型组比较,麦门冬汤中、高剂量组和醋酸泼尼松组大鼠 FVC、PEF、Cydn 均明显升高(P 均<0.05),RL、肺泡炎评分、肺纤维化评分均明显降低(P 均<0.05);肺组织中 E-cad 阳性表达平均光密度值及 E-cad mRNA 相对表达量均明显升高(P 均<0.05),α-SMA 和 CollagenⅠ阳性表达平均光密度值、微血管密度及α-SMA、Collagen Ⅰ、VEGF、FGF2 mRNA 相对表达量均明显降低(P 均<0.05).结论 麦门冬汤可明显改善特发性肺纤维化大鼠肺间质纤维化程度,减轻肺组织损伤,提高肺功能,其机制可能与抑制肺组织上皮-间质转化及血管新生有关.

Objective It is to investigate the effect and its possible mechanism of Maimendong Decoction(MD)in the treatment of idiopathic pulmonary fibrosis(IPF).Methods Sixty Wistar rats were divided into 6 groups,with 10 rats in each group.The rats of all groups except for the normal group were given bleomycin(BLM)via intratracheal injection to establish models of IPF.After successful modeling,the low-dose,medium-dose and high dose groups of MD were given MD 7.5 g/kg,15 g/kg,30 g/kg via gavage,the prednisone acetate group was given prednisone acetate 6.25 mg/kg via gavage,the normal group and model group were given the same amount of normal saline via gavage,all once daily,contin-uously treated for 3 weeks.The lung function parameters[forced vital capacity(FVC),peak expiratory flow(PEF),total lung resistance(RL)and dynamic compliance(Cydn)]of rats in each group were detected,the lung tissue pathology was observed using HE staining and Masson's staining,the positive expressions of E-cadherin(E-cad),α-smooth muscle actin(α-SMA)and Collagen Ⅰ,as well as microvascular density(CD34 labeling)were detected by immunohistochemistry,the mRNA expressions of E-cad,α-SMA,Collagen Ⅰ,vascular endothelial growth factor(VEGF)and fibroblast growth factor 2(FGF2)in lung tissue were detected by Real-time quantitative PCR.Results Compared with the control group,the FVC,PEF and Cydn of rats in the model group were significantly reduced(all P<0.05),while the RL,alveolitis scores,and pulmonary fibrosis scores of the rats were significantly increased(all P<0.05);the mean optical density of E-cad positive expression and the relative expression of E-cad mRNA in lung tissue of the rats were both significantly reduced(all P<0.05),while the mean optical densities of α-SMA and Collagen Ⅰ positive expressions,microvascular density,and the relative mRNA expressions of α-SMA,Collagen Ⅰ,VEGF,and FGF2 in lung tissue of the rats were all significantly increased(all P<0.05).Compared with the model group,the FVC,PEF and Cydn of rats in the medium-dose and high dose groups of MD and prednisone acetate group were significantly increased(all P<0.05),while the RL,alveolitis scores,and pulmonary fibrosis scores of the rats were significantly decreased(all P<0.05);the mean optical density of E-cad positive expression and the relative expression of E-cad mRNA in lung tissue of the rats were both significantly in-creased(all P<0.05),while the mean optical densities of α-SMA and Collagen Ⅰ positive expressions,microvascular density,and the relative mRNA expressions of α-SMA,Collagen Ⅰ,VEGF,and FGF2 in lung tissue of the rats were all significantly decreased(all P<0.05).Conclusion MD can significantly alleviate the degree of pulmonary interstitial fibro-sis and lung tissue damage and improve lung function of rats with IPF,and its mechanism may be related to inhibiting epi-thelial-mesenchymal transition and angiogenesis.
