足三里穴位注射黄芪注射液对机械通气患者膈肌功能及撤机的影响OA
Effects of acupoint injection at Zusanli acupoint with astragalus injection on diaphragmatic function and weaning in patients treated with mechanical ventilation
目的 探讨足三里穴位注射黄芪注射液对机械通气患者膈肌功能及撤机的影响.方法 选取2021 年1 月—2023 年 12 月上海市宝山区中西医结合医院重症医学科(ICU)收治的112 例机械通气合并膈肌功能障碍的重症肺炎患者为研究对象,采用随机数字表法随机分为试验组和对照组,每组56 例.对照组采取有创机械通气结合内科综合治疗,试验组在对照组治疗基础上给予足三里穴位注射黄芪注射液,2 组疗程均为7 d.比较2 组治疗前、治疗7d 后的膈肌增厚率(DTF)、膈肌移动度(DE)、氧合指数(OI)、呼吸浅快指数(RSBI)、肺顺应性,统计 2 组机械通气时间、7 d 撤机成功率、不良反应发生率.结果 治疗7d 后2 组DTF、DE、OI 均较治疗前明显升高(P均<0.05),且试验组各指标均明显高于对照组(P 均<0.05);2 组 RSBI 均较治疗前明显降低(P均<0.05),且试验组 RSBI 明显低于对照组(P<0.05);2 组肺顺应性均较治疗前明显升高(P 均<0.05),且试验组肺顺应性明显高于对照组(P<0.05).试验组机械通气时间明显短于对照组(P<0.05),7 d 撤机成功率明显高于对照组(P<0.05).2 组不良反应发生率比较差异无统计学意义(P>0.05).结论 足三里穴位注射黄芪注射液可增强机械通气患者膈肌功能,缩短机械通气时间,改善呼吸功能,提高撤机成功率,且安全.
Objective It is to investigate the effects of acupoint injection at Zusanli acupoint with astragalus injection on diaphragmatic function and weaning in patients treated with mechanical ventilation.Methods A total of 112 patients with severe pneumonia admitted to the Department of ICU of Shanghai Baoshan District Hospital of Integrated Traditional and Western Medicine from January 2021 to December 2023 and suffered from diaphragmatic dysfunction during mechanical ventilation were enrolled,and were randomly divided into experimental group and control group by random number table method,with 56 patients in each group.The control group was treated with invasive mechanical ventilation combined with comprehensive medical therapy,while the experimental group was treated with acupoint injection at Zusanli acupoint with astragalus injection on the basis of therapy in the control group,both groups were treated for 7 days.The diaphragmatic thickening fraction(DTF),diaphragmatic excursion(DE),oxygenation index(OI),respiratory shallow breathing index(RSBI)and lung compliance were compared between the two groups before treatment and after 7 days of treatment.Addi-tionally,the duration of mechanical ventilation,the successful rate of weaning on the 7th day and the incidence of adverse reactions in both groups were calculated.Results After 7 days of treatment,the DTF,DE,OI and lung compliance in both groups were significantly higher than those before treatment(all P<0.05),and all these indicators in the experimental group were significantly higher than those in the control group(all P<0.05);the RSBI in both groups were significantly lower than those before treatment(both P<0.05),and the RSBI in the experimental group was significantly lower than that in the control group(P<0.05).The duration of mechanical ventilation in the experimental group was significantly shorter,and the successful rate of weaning on the 7th day was significantly higher than those in the control group(both P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Acupoint injection at Zusanli acupoint with astragalus injection can enhance diaphragmatic function,shorten the duration of mechanical ventilation and improve respiratory function of patients treated with mechanical ventila-tion,and can increase the success rate of weaning with good safety.
