目的 探讨固肾泄浊和络方对糖尿病肾病大鼠肾脏的保护作用及对核因子-κB(NF-κB)/Wnt/β-连环蛋白(β-catenin)信号通路的影响.方法 取 72 只雄性 SD 大鼠,随机选择12 只作为正常组,其余60 只采用1%链脲佐菌素(STZ)溶液一次性左侧腹腔注射方法建立糖尿病肾病模型.将50 只造模成功大鼠随机分为模型组、厄贝沙坦组、固肾泄浊和络方低剂量组、固肾泄浊和络方中剂量组、固肾泄浊和络方高剂量组,每组10 只.厄贝沙坦组按照0.0781 g/kg 剂量给予厄贝沙坦灌胃,固肾泄浊和络方低、中、高剂量组分别给予浓度为0.53 g/mL、1.06 g/mL、2.12 g/mL的固肾泄浊和络方药液灌胃,模型组及正常组给予生理盐水灌胃,灌胃量均为10 mL/(kg·d),均1 次/d,连续灌胃6 周.测定大鼠灌胃前及末次灌胃后体重、随机血糖、24 h 尿蛋白定量,检测末次灌胃后血肌酐、血尿素氮水平;HE 及 PAS 染色进行肾组织病理观察;qPCR 法和 Western blot 法分别检测肾组织中NF-κB、Ⅳ型胶原、Wnt4、β-catenin、基质金属蛋白酶(MMP)-7、MMP-3 mRNA及蛋白表达情况.结果 与正常组比较,模型组大鼠体重明显减轻(<0.05),随机血糖、血肌酐、血尿素氮及24 h 尿蛋白定量均明显升高(P 均<0.05);肾小球基底膜增厚,系膜基质增多,间质炎细胞浸润,肾脏纤维化改变;肾组织中 NF-κB、Ⅳ型胶原、Wnt4、β-catenin、MMP-7、MMP-3 mR-NA 及蛋白相对表达量均明显升高(P 均<0.05).与模型组比较,固肾泄浊和络方中、高剂量组及厄贝沙坦组大鼠体重明显增加(P 均<0.05),随机血糖、血肌酐、血尿素氮及24 h 尿蛋白定量均明显降低(P 均<0.05);系膜基质减少,间质炎细胞浸润、肾脏纤维化均缓解;肾组织中 NF-κB、Ⅳ型胶原、Wnt4、β-catenin、MMP-7、MMP-3 mRNA 及蛋白相对表达量均明显降低(P 均<0.05);且固肾泄浊和络方高剂量组上述各指标(除体重、随机血糖、血肌酐、血尿素氮)较其他给药组变化更明显(P 均<0.05).结论 固肾泄浊和络方能明显减轻糖尿病肾病大鼠肾功能损伤和肾脏纤维化,抑制炎症反应,其可能通过调控 NF-κB/Wnt/β-catenin 信号通路,下调 MMP-7、MMP-3表达,减少Ⅳ型胶原过度沉积而发挥肾脏保护作用.

Objective It is to investigate the protective effect of Gushen Xiezhuo Heluo prescription on the kidney of rats with diabetic kidney disease and its effect on nuclear factor kappa-B(NF-κB)/Wnt/β-catenin pathways.Methods Seventy-two male SD rats were taken,12 of which were randomly selected as normal group,and the remaining 60 rats were given 1%streptozotocin(STZ)solution by a single intraperitoneal injection into the left side to establish models of diabetic kidney disease.50 successfully model rats were randomly divided into model group,irbesartan group,low-dose,medium-dose and high-dose groups of Gushen Xiezhuo Heluo prescription,with 10 rats in each group.The irbesartan group was ad-ministered irbesartan via gastric gavage at a dose of 0.0781 g/kg,while the low-dose,medium-dose and high-dose groups of Gushen Xiezhuo Heluo prescription were administered the formula solution via gastric gavage at concentrations of 0.53 g/mL,1.06 g/mL,and 2.11 g/mL,respectively,the control group and normal group were administered normal saline via gavage,all once daily,continuously treated for 6 weeks.The general condition of the rats was observed,their body weight,random blood glucose,and 24-hour urine protein(24hUP)were measured before the first gavage and after the final gavage,the se-rum levels of creatinine(SCr)and blood urea nitrogen(BUN)were measured after the final gavage;the renal tissue pa-thology was observed by HE staining and PAS staining;the mRNA and protein expressions of NF-κB,type Ⅳ collagen,Wnt4,β-catenin,matrix metalloproteinase(MMP)-7,and MMP-3 in renal tissue were detected by qPCR and Western blot assays,respectively.Results Compared with the normal group,the body weight of rats in the model group was decreased(P<0.05),their random blood glucose,SCr,BUN and 24hUP were significantly increased(all P<0.05);the patholog-ical changes such as thickened basement membrane,increased mesangial matrix,infiltration of interstitial cells and renal fi-brosis were found;the mRNA and protein expressions of NF-κB,type Ⅳ collagen,Wnt4,β-catenin,MMP-7 and MMP-3 in renal tissue were increased(all P<0.05).Compared with the model group,the body weights of rats in the medium-dose and high-dose groups of Gushen Xiezhuo Heluo prescription and irbesartan group were decreased(all P<0.05),their random blood glucose,SCr,BUN and 24hUP were significantly decreased(all P<0.05);the mesangial matrix was de-creased,the infiltration of interstitial cells and renal fibrosis were alleviated;the mRNA and protein expressions of NF-κB,type Ⅳ collagen,Wnt4,β-catenin,MMP-7 and MMP-3 in renal tissue were decreased(all P<0.05),and the improve-ments of these indicators except for the body weight,random glucose,SCr and BUN in the high-dose group of Gushen Xiezhuo Heluo prescription were more significant than those in the other medicated groups(all P<0.05).Conclusion Gushen Xiezhuo Heluo prescription can significantly alleviate renal dysfunction and renal fibrosis,and inhibit inflammatory responses in rats with diabetic nephropathy.It may exert its nephroprotective effects via regulating the NF-κB/Wnt/β-cate-nin signaling pathway,down-regulating the expressions of MMP-7 and MMP-3,and reducing the excessive deposition of type Ⅳ collagen.
