AI-Powered Rib Fracture CT Diagnostic System:Development,Integration and Application
本研究探索胸部CT肋骨骨折人工智能(AI)辅助诊断系统与医学影像存档与通信系统(PACS)的集成方法,以显著提升肋骨骨折诊断效率与准确性.研究构建了高效的胸部CT肋骨骨折AI辅助诊断系统,并设计了该系统与PACS 的集成接口.共纳入 187 例胸部外伤患者的胸部CT 影像,所有患者均于胸部外伤当日及伤后 1~3 个月完成两次CT 扫描.以第二次胸部 CT 复查出现明确骨痂作为肋骨骨折诊断金标准,对比分析首次胸部 CT 检查中人工诊断与 AI 辅助诊断结果的一致性及诊断耗时差异.结果显示,集成后的 AI 辅助诊断系统可提升肋骨骨折诊断效率,平均诊断时间缩短 15.12%;同时提高了肋骨骨折诊断效能:肋骨完全骨折诊断灵敏度平均提升 5.06%,阳性预测值平均提升 1.51%;肋骨不完全骨折诊断灵敏度平均提升 16.45%,阳性预测值平均提升 4.65%,均显著优于传统人工诊断方法.该创新集成方案具备重要临床应用价值与广阔应用前景,有望在临床诊疗中发挥重要作用.
This study explores the integration scheme of an Artificial Intelligence(AI)-assisted diagnostic system for rib fractures on chest CT with the Picture Archiving and Communication System(PACS),aiming to significantly improve the efficiency and accuracy of rib fracture diagnosis.An efficient AI-assisted diagnostic system for rib fractures on chest CT is constructed,and an integration interface between the system and PACS is designed.A total of 187 patients with chest trauma are enrolled,and all patients undergo two CT scans on the day of chest trauma and within 1 to 3 months after injury.The presence of definite callus on the second follow-up chest CT is used as the gold standard for the diagnosis of rib fractures.The consistency between manual diagnosis and AI-assisted diagnosis based on the first chest CT,as well as the difference in diagnostic time,are compared and analyzed.The integrated AI-assisted diagnostic system improves the diagnostic efficiency of rib fractures,with the average diagnostic time shortened by 15.12%.Meanwhile,the system enhances the diagnostic efficacy of rib fractures that the average sensitivity for diagnosing complete rib fractures is increased by 5.06%,and the average positive predictive value is increased by 1.51%.The average sensitivity for diagnosing incomplete rib fractures is increased by 16.45%,and the average positive predictive value is increased by 4.65%,both of which are significantly superior to traditional manual diagnosis.This innovative integration scheme has important clinical application value and broad application prospects,and is expected to play an important role in clinical practice.
