目的 分析纵隔支气管囊肿(MBC)的多层螺旋 CT(MSCT)影像学特征,指导临床诊断及鉴别诊断.方法 回顾性分析联勤保障部队第 940 医院的 31 例临床资料完整、经手术病理证实的 MBC 的 MSCT 特征,对病灶在纵隔的定位、形态、密度、边缘、囊壁、强化程度等进行观察及分型,并与既往文献进行对比分析.结果 首次发现前纵隔为 MBC新优势分布区(13/31,41.9%),证实定位与密度显著相关(P=0.032),其中低密度占比 53.8%(OR=4.08).三维形态分析显示96.8%(30/31)病例具有特征性"类椭圆体"形态,48.4%(15/31)病例显示"类椭圆体-血管平行征"复合征象,长轴与邻近血管走行一致.增强扫描无强化.结论 综合解剖-密度关联、椭圆体形态指数、血管平行征等特征组合,可提升MBC 诊断准确率.

Objective To analyze the multi-slice spiral CT(MSCT)imaging features of mediastinal bronchial cysts(MBC)to guide clinical diagnosis and differential diagnosis.Methods A retrospective analysis was conducted on the MSCT features of 31 ca-ses of MBC with complete clinical data and confirmed by surgery and pathology.The location,morphology,density,margin,cyst wall,degree of enhancement,etc.of the lesions in the mediastinum were observed and classified,and compared with previous literatures.Results For the first time,it was found that the anterior mediastinum was a new predominant distribution area of MBC(13/31,41.9%),and the location was significantly correlated with the density(P=0.032),with a low-density proportion of 53.8%(OR=4.08).Three-dimensional morphological analysis showed that 96.8%(30/31)of the cases had a characteristic"ellipsoid-like"mor-phology,and 48.4%(15/31)of the cases showed a"ellipsoid-like-parallel vascular sign"composite sign,with the long axis con-sistent with the course of adjacent vessels.There was no enhancement on enhanced scanning.Conclusions The combination of fea-tures such as anatomical-density association,ellipsoid morphological index and parallel vascular sign can improve the diagnostic accura-cy of MBC.
