目的 利用孟德尔随机化(Mendelian randomization,MR)方法,探究多种代谢指标与乳腺癌发病风险之间的潜在因果关系.方法 本研究共纳入13种代谢指标为暴露因素,其遗传工具变量来源于相应的全基因组关联研究数据库(GWAS),结局数据为乳腺癌来自芬兰数据库(FinnGen).筛选符合MR三大核心假设的单核苷酸多态性(SNP)作为工具变量,并以逆方差加权法(IVW)为主要分析方法、结合MR-Egger回归、加权中位数法、加权模式及简单模式综合评估暴露与结局的因果关系.采用多效性检验、异质性检验及"留一法"敏感性分析以确保MR结果的可靠性.结果 在13种暴露因素中,载脂蛋白A-V(apoA-V)、低密度脂蛋白中甘油三酯与总脂质的比例(TG/LDL total lipid)、高血压、舒张压(DBP)、体质量指数(BMI)以及经BMI校正的腰臀比(WHRadjBMI)为乳腺癌发生的保护因素;胰岛素原和胰岛素受体为乳腺癌发生的危险因素;甘油三酯(TG)、载脂蛋白B(apoB)、总胆固醇(TC)、低密度脂蛋白胆固醇水平(LDL-C)及高密度脂蛋白胆固醇(HDL-C)均未发现与乳腺癌存在统计学意义的因果关系.敏感性分析结果显示,工具变量不存在显著的水平多效性、异质性,且MR估计值不受单个SNP的影响.结论 胰岛素原和胰岛素受体与乳腺癌发病风险升高有关,apoA-V、TG/LDL total lipid、高血压、DBP、BMI、WHRadjBMI与乳腺癌发病风险降低有关,提示机体不同代谢指标与乳腺癌发病风险之间存在差异.

Objective To explore the potential causal relationship between multiple metabolic parameters and the risk of breast cancer using Mendelian randomization(MR)method.Methods A total of 13 metabolic parameters were enrolled as exposure factors in this study.The genetic instrumental variables for these factors were derived from the corresponding Genome-Wide Association Study(GWAS)databases,and the outcome data for breast cancer were obtained from the FinnGen database.Single nucleotide polymor-phisms(SNPs)that conformed to the three core assumptions of MR were screened as instrumental variables.The inverse variance weighted(IVW)method was adopted as the primary analytical approach,and MR-Egger regression,the weighted median(WM)method,the weighted mode method and the simple mode method were combined to comprehensively evaluate the causal relationships between exposures and outcome.Pleiotropy test,heterogeneity test and leave-one-out sensitivity analysis were performed to ensure the reliability of MR results.Results Among the 13 exposure factors,apolipoprotein A-V(apoA-V),the ratio of triglycerides to total lipids in low-density lipoprotein(TG/LDL total lipid),hypertension,diastolic blood pressure(DBP),body mass index(BMI),and waist-hip ratio adjusted for BMI(WHRadjBMI)were identified as protective factors for breast cancer.Proinsulin and insulin receptor(INSR)were confirmed as risk factors for breast cancer.No causal relationship with statistical significance was found between breast cancer and the remaining parameters,including triglycerides(TG),apoB,total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),and high-density lipoprotein cholesterol(HDL-C).The results of sensitivity analyses showed that there was no significant horizontal pleiotropy or heterogeneity in the selected instrumental variables,and the MR effect estimates were not affected by any indi-vidual SNP.Conclusion Proinsulin and insulin receptor are associated with an elevated risk of breast cancer,while apoA-V,TG/LDL total lipid,HTN,DBP,BMI and WHRadjBMI are correlated with a reduced risk of breast cancer.These findings indicate that there are differential associations between various metabolic parameters in human body and the risk of breast cancer.
