目的 分析兰州地区 MCS+血细胞分离机血小板采集冲红的原因及应对措施.方法 选取兰州地区 2022 年12 月至 2024 年 12 月使用 MCS+Haemontics 血细胞分离机、Haemontics REF 0996 E-00/996 E2-00 管路(美国血液技术公司)采集血小板的献血者 90 例,依据冲红情况划分为两组,未冲红组 70 例,未冲红;冲红组 20 例,冲红.统计两组采血前血细胞计数.结果 90 例献血者中,冲红 20 例,冲红发生率为 22.22%.冲红组采血前血细胞计数中的红细胞计数(RBC)、红细胞平均体积(MCV)、红细胞比容(Hct)、血红蛋白(Hb)、平均血红蛋白含量(MCH)、红细胞平均血红蛋白浓度(MCHC)均低于未冲红组(P<0.05),但两组白细胞计数(WBC)、血小板计数(PLT)、血小板体积分布宽度(PDW)、红细胞平均分布宽度(RDW)、外周血血小板平均体积(MPV)、大血小板比例(P-LCR)差异不显著(P>0.05).结论 兰州地区 MCS+血细胞分离机采集血小板冲红的原因为采血前血细胞计数中的 RBC、Hb、Hct、MCV、MCH、MCHC 数值偏低,血站采血科室需针对上述原因采取有效的应对措施,如将采血速度适当降低并保持稳定,使血细胞在离心过程中各层之间形成充分分层,血细胞分离机的光学探测器能够有效识别血小板,进而在血浆从管路感知器通过时提升血小板采集效率,降低冲红发生率,最终使临床输血安全得到有效保证.

Objective:To analyze the causes and countermeasures of red blood platelet collection by MCS++blood cell separator in Lanzhou area.Methods:A total of 90 blood donors who underwent plateletpheresis in Lanzhou area from December 2022 to December 2024 were enrolled in this study.Platelet collection was performed using the MCS+blood cell separator(Haemonetics,USA)with Haemonetics REF 0996 E-00/996 E2-00 tubing sets.The subjects were divided into two groups according to whether red flashing occurred during collection:70 cases in the non-red-flashing group(no red flashing)and 20 cases in the red-flashing group(red flashing occurred).Pre-collection blood cell counts were compared between the two groups.Results:Among 90 blood donors,20 cases were red-washed,and the incidence of red-washed was 22.22%.The red blood cell count(RBC)、mean red blood cell volume(MCV)、red blood cell volume(Hct)、hemoglobin(Hb)、mean hemoglobin content(MCH)and mean red blood cell hemoglobin concentration(MCHC)in the red blood flushing group were lower than those in the red blood flushing group(P<0.0 5).However,there were no significant differences in white blood cell count(WBC)、platelet count(PLT)、platelet volume distribution width(PDW)、average red blood cell distribution width(RDW)、average peripheral blood platelet volume(MPV)and large platelet ratio(P-LCR)between the two groups(P>0.05).Conclusion:The reason of red blood platelet collected by MCS+blood cell separator in Lanzhou area is that the values of RBC、Hb、Hct、MCV、MCH and MCHC in blood cell count before blood collection are low.The blood collection department of blood station should take effective measures against the above reasons,such as reducing the blood collection speed appropriately and keeping it stable,so that blood cells can be fully stratified between lay-ers during centrifugation,and the optical detector of blood cell separator can effectively identify platelets,thus im-proving the efficiency of platelet collection and reducing red blood platelet when plasma passes through the pipeline sensor.
