Effect And Learning Curve analysis of Smartphone Analog Navigation in Minimally Invasive Cerebral Hemorrhage Clearance
目的 探讨智能手机模拟导航用于脑出血微创清除术的效果及学习曲线.方法 选取 2023 年 3 月至 2025 年 3月温州市中西医结合医院收治的脑出血患者 116 例为研究对象,采用计算机随机数字表法分为两组,对照组 58 例予以传统 CT 导航下脑出血微创清除术,观察组 58 例予以智能手机模拟导航脑出血微创清除术.比较两组主要结局与次要结局,同时采用 CUSUM回归模型绘制智能手机模拟导航的学习曲线.以手术时间的学习曲线顶点为界,将观察组中 58 例患者分为学习组和掌握组,比较手术效果.结果 观察组手术时间、引流管拔除时间、ICU 及住院时间均较对照组短,术中出血量及定位误差较对照组小,血肿清除率较对照组高(P<0.05);基于手术时间的学习曲线拐点为第 15 例.以第 15 例为界,观察组分为学习组(第 1~15 例)与掌握组(第 16~58 例),学习组手术时间(98.65±24.26)min,较掌握组(84.25±20.65)min 长(P<0.05).结论 智能手机模拟导航用于脑出血微创清除术的效果较为满意,跨越学习曲线达到熟练掌握阶段所需要累积的手术例数约为 15 例.
Objective:To explore the effect and learning curve of smart phone simulation navigation in minimally invasive re-moval of cerebral hemorrhage.Methods:116 patients with cerebral hemorrhage admitted to Wenzhou Hospital of Integrat-ed Traditional Chinese and Western Medicine from March 2023 to March 2025 were selected as the research object,and were divided into two groups by computer random number table method.The control group(58 cases)was treated with traditional CT navigation minimally invasive surgery for cerebral hemorrhage,and the observation group(58 cases)was treated with smart phone simulation navigation minimally invasive surgery for cerebral hemorrhage.Compare the main outcome and the secondary outcome of the two groups,and draw the learning curve of smart phone simulation navigation by using CUSUM regression model.58 patients in the observation group were divided into learning group and mastering group with the vertex of the learning curve of operation time as the boundary,and the operation effect was compared.Re-sults:The operation time,drainage tube removal time,ICU stay and hospitalization time in the observation group were shorter than those in the control group,and the intraoperative blood loss and positioning error were smaller than those in the control group,and the hematoma clearance rate was higher than that in the control group(P<0.05).The inflection point of learning curve based on operation time was the 15th case.Taking the 15th case as the boundary,the observation group was divided into study group(cases 1~15)and master group(cases 16~58).The operation time of study group was(98.65±24.26)min,which was longer than that of master group(84.25±20.65)min(P<0.05).Conclusion:The effect of smart phone simulation navigation in minimally invasive removal of cerebral hemorrhage is satisfactory,and the cumulative number of surgical cases required to cross the learning curve to master the stage is about 15 cases.
