基于HSC"线粒体异常激活"探讨放疗相关骨髓抑制"相火妄动"病机及其辨治思路OA
Pathogenesis Exploration of"Ministerial Fire Hyperactivity"in Radiotherapy-Related Myelosuppression Based on HSC"Abnormal Mitochondrial Activation"and Its Diagnostic-Treatment Strategies
骨髓造血干细胞(Hematopoietic stem cells,HSC)的多向分化(动员)与自我更新(稳态维持)是造血平衡的基础,放疗诱导其"线粒体异常激活",进而导致HSC"动员-稳态维持"失衡是放疗相关骨髓抑制的关键.放射线与中医"火毒热邪"的致病特点相符,随着放射剂量的累积,劫耗阴精,损及髓源,引动相火,导致"相火妄动"发为放疗相关骨髓抑制.基于骨髓HSC及线粒体分别作为中医"肾精"和"相火"的微观具象,总结认为HSC"线粒体异常激活"与"相火妄动"皆以肾精亏损为始,虚亢致害为渐,正虚邪踞为终.因此,本文聚焦HSC线粒体异常激活,系统阐析放疗相关骨髓抑制"相火妄动"病机的科学内涵,并基于"固本、伏火、涤髓"治法阐明从肾论治放疗相关骨髓抑制对造血功能维持的重要意义.
The multidirectional differentiation(mobilization)and self-renewal(mobilization-maintenance)of bone marrow hematopoietic stem cells(HSCs)ensure hematopoietic homeostasis.Radiotherapy-induced"mitochondrial dysregulation"in HSCs disrupts the"mobilization-maintenance"equilibrium,serving as a key mechanism underlying radiotherapy-related myelosuppression.In traditional Chinese medicine(TCM),radiation is categorized as a"fire-toxin heat pathogen".With cumulative radiation doses,it depletes Yin essence,damages marrow sources,and triggers"Ministerial Fire Hyperactivity",ultimately manifesting as myelosuppression.Based on the microcosmic correspondence between HSCs/mitochondria and TCM concepts of Kidney Essence/"Ministerial Fire Hyperactivity",we propose that both HSC"mitochondrial dysregulation"and"Ministerial Fire Hyperactivity"originate from Kidney Essence deficiency,progress through deficiency-induced hyperactivity,and culminate in a pathological state of deficient healthy qi and entrenched pathogenic factors.This study elucidates the scientific connotation of the"Ministerial Fire Hyperactivity"pathomechanism in radiation-induced myelosuppression through the lens of HSC mitochondrial dysregulation.Furthermore,guided by the therapeutic principles of"consolidating the root,subduing fire,and purging marrow toxins",we highlight the significance of kidney-focused treatment strategies in maintaining hematopoietic function during radiotherapy.
