A preliminary investigation of the synergistic effects of breathing training and transcutaneous auricular vagus nerve stimulation using resting-state functional magnetic resonance imaging

目的 运用静息态功能磁共振成像技术(resting-state functional magnetic resonance imaging,rs-fMRI)观察调息联合经皮耳穴迷走神经刺激(transcutaneous auricular vagus nerve stimulation,taVNS)的协同效应.方法 纳入20名健康者志愿者,采用随机交叉对照试验分为2组,一组先接受taVNS即刻干预30 min,后接受调息联合 taVNS 干预 30 min,两次干预时间间隔至少 1 周;另一组反之.两组均在每次干预前后进行rs-fMRI 扫描,以前扣带回的 5 个不同部分作为感兴趣区观察两种干预方式前后的全脑功能连接(functional connectivity,FC)变化.结果 调息联合 taVNS即刻刺激后右侧背侧前扣带回与左侧膝前部前扣带回以及右侧膝下前扣带回与左侧岛叶 FC 降低;taVNS 在即刻降低右侧背侧前扣带回和右侧角回 FC 值;以上差异脑区均经高斯随机场校正(voxel P<0.01,cluster P<0.05);事后分析显示两种干预方式作用趋势一致.结论 调息可协同促进 taVNS 在情绪相关特别是焦虑情绪相关脑区的脑效应,初步印证调息联合 taVNS 存在协同增效作用,提示联合调息可能是提高taVNS调节情绪效应的潜在优化的方向之一.

Objective To observe the synergistic effects of breathing training and transcutaneous auricular vagus nerve stimulation(taVNS)using resting-state functional magnetic resonance imaging(rs-fMRI).Method Twenty healthy volunteers were recruited and divided into two groups to conduct a randomized crossover-controlled trial.One group first received an immediate intervention of taVNS for 30 min,followed by a 30-min intervention of breathing training combined with taVNS,with an interval of at least 1 week between the two sessions.The other group received the interventions in reverse order.For both groups,rs-fMRI was performed before and after each intervention.The five distinct subregions of the anterior cingulate cortex were selected as regions of interest(ROIs)to examine changes in whole-brain functional connectivity(FC)before and after the two types of interventions.Result Following immediate intervention of breathing training combined with taVNS,the functional connectivity(FC)between the right dorsal anterior cingulate cortex and the left pregenual anterior cingulate cortex,as well as between the right subgenual anterior cingulate cortex and the left insula,was decreased;immediate intervention of taVNS reduced the FC between the right dorsal anterior cingulate cortex and the right angular gyrus.All brain regions showing significant differences described above survived Gaussian random field correction(voxel-level P<0.01,cluster-level P<0.05).Post-hoc analysis revealed consistent trends in the effects of the two types of interventions.Conclusion Breathing training can synergistically enhance the brain effects of taVNS on emotion-associated brain regions,particularly those related to anxiety.This study provides preliminary evidence for the synergistic effects between breathing training and taVNS,suggesting that integrating breathing training may be one of the potential approaches for enhancing the emotional regulatory effect of taVNS.
