靳三针联合中药治疗肝胃不和型功能性消化不良的疗效观察OA
Efficacy of Jin's three-needle therapy combined with Chinese herbal medicine in the treatment of functional dyspepsia with liver-stomach disharmony pattern
目的 观察靳三针联合柴胡疏肝散合左金丸治疗肝胃不和型功能性消化不良的临床疗效及其对患者胃肠激素和脑肠肽水平的影响.方法 选取 80 例肝胃不和型功能性消化不良患者,按随机数字表法分为对照组和观察组,每组 40例.对照组口服柴胡疏肝散合左金丸,观察组在对照组口服中药基础上联合靳三针治疗.比较两组临床疗效,观察两组治疗前后中医证候积分、临床症状评分、功能性消化不良生存质量量表(functional dyspepsia quality of life,FDQOL)评分、4 h胃排空率、血清胃肠激素[胃动素(motilin,MTL)、胃泌素(gastrin,GAS)、胆囊收缩素(cholecystokinin,CCK)和血管活性肠肽(vasoactive intestinal peptide,VIP)]及脑肠肽[5-羟色胺(5-hydroxytryptamine,5-HT)和 P 物质(substance P,SP)]水平的变化.结果 观察组总有效率高于对照组(P＜0.05).治疗后,两组中医证候积分、临床症状评分及血清 CCK、VIP、5-HT 和 SP 水平均较治疗前降低(P＜0.05),且观察组均低于对照组(P＜0.05).治疗后,两组4 h 胃排空率、血清MTL 和GAS水平及 FDQOL评分均较治疗前升高(P＜0.05),且观察组均高于对照组(P＜0.05).结论 靳三针联合柴胡疏肝散合左金丸可显著改善肝胃不和型功能性消化不良患者临床症状和生活质量,临床疗效优于单纯中药治疗,其机制可能与调节胃肠激素和脑肠肽水平以及促进胃排空相关.
Objective To observe the clinical efficacy of Jin's three-needle therapy combined with Chai Hu Shu Gan powder and Zuo Jin pill in the treatment of functional dyspepsia with liver-stomach disharmony pattern,and its effects on gastrointestinal hormones and brain-gut peptides.Method A total of 80 patients with functional dyspepsia with liver-stomach disharmony pattern were selected and divided into a control group and an observation group according to a random number table method,with 40 cases in each group.The control group was treated with oral Chai Hu Shu Gan powder combined with Zuo Jin pill,while the observation group received Jin's three-needle therapy in addition to the oral herbal treatment used in the control group.The clinical efficacy was compared between the two groups.Changes in traditional Chinese medicine(TCM)pattern scores,clinical symptom scores,functional dyspepsia quality of life(FDQOL)scores,4 h gastric emptying rate,serum gastrointestinal hormones[motilin(MTL),gastrin(GAS),cholecystokinin(CCK),and vasoactive intestinal peptide(VIP)],and brain-gut peptides[5-hydroxytryptamine(5-HT)and substance P(SP)]were observed before and after treatment.Result The total effective rate in the observation group was higher than that in the control group(P＜0.05).After treatment,TCM pattern scores,clinical symptom scores,and serum levels of CCK,VIP,5-HT,and SP in both groups decreased compared with those before treatment(P＜0.05),and those in the observation group were lower than those in the control group(P＜0.05).After treatment,the 4 h gastric emptying rate,serum levels of MTL and GAS,and FDQOL scores in both groups increased compared with those before treatment(P＜0.05),and those in the observation group were higher than those in the control group(P＜0.05).Conclusion Jin's three-needle therapy combined with Chai Hu Shu Gan powder and Zuo Jin pill can significantly improve clinical symptoms and quality of life in patients with functional dyspepsia with liver-stomach disharmony pattern.Its clinical efficacy is superior to that of herbal medicine alone,and the mechanism may be related to regulating gastrointestinal hormones and brain-gut peptides and promoting gastric emptying.
