目的 观察肌电生物反馈疗法联合瑞士球本体感觉训练对全膝关节置换术(TKA)术后患者的康复效果.方法 前瞻性纳入TKA治疗患者80例,采用随机数字表法分为对照组、观察组各40例.对照组TKA术后采用瑞士球本体感觉训练,观察组加用肌电生物反馈治疗,持续治疗12周.治疗后比较两组膝关节功能、运动与平衡功能、疼痛程度、核心肌力情况、日常生活能力及治疗期间并发症情况.结果 与治疗前比较,治疗后两组纽约特种外科医院膝关节评分(HSS)、Berg平衡量表(BBS)评分、Fugl-Meyer评定量表(FMA)评分、屈肌群峰力矩(PT)、伸肌群PT、Lysholm膝关节评分、Barthel指数(BI)均高,视觉模拟量表(VAS)评分低(P均<0.05);与对照组比较,观察组HSS、BBS评分、FMA评分、屈肌群PT、伸肌群PT、Lysholm膝关节评分、BI均高,VAS评分低(P均<0.05).治疗期间两组并发症发生率比较差异无统计学意义(P>0.05).结论 肌电生物反馈疗法联合瑞士球本体感觉训练对TKA术后患者的康复效果较好,有助于患者膝关节功能恢复及下肢肌群肌力改善.

Objective To observe the rehabilitation effect of electromyographic biofeedback therapy combined with Swiss ball proprioceptive training on patients after total knee arthroplasty(TKA).Methods Eighty patients undergoing TKA were prospectively included,and were divided into the control group and observation group using the random number table method,with 40 cases in each group.Patients in the control group adopted Swiss ball proprioceptive training,while patients in the observation group additionally adopted electromyographic biofeedback therapy.The treatment lasted for 12 weeks.We compared knee joint function,motor and balance functions,pain level,core muscle strength,daily living abili-ty,and complications during the treatment between the two groups.Results Compared with those before treatment,the scores of Hospital for Special Surgery(HSS),Berg Balance Scale(BBS),Fugl-Meyer Assessment(FMA),the peak torque(PT)of the flexor muscles,PT of the extensor muscles,Lysholm knee joint score,and Barthel Index(BI)in both groups after treatment were all higher,while the visual analogue scale(VAS)score was lower(all P<0.05).Compared with those in the control group,the scores of HSS,BBS,FMA,PT of the flexor muscles,PT of the extensor muscles,Ly-sholm knee joint score,and BI in the observation group were all higher,while the VAS score was lower(all P<0.05).However,there was no significant difference in the incidence of complications between two groups(P>0.05).Conclusion The combination of electromyographic biofeedback therapy and Swiss ball proprioceptive training has a better rehabilita-tion effect on patients after TKA,and is beneficial for the recovery of knee joint function and the improvement of lower limb muscle strength.
