免疫检查点抑制剂在妊娠滋养细胞肿瘤中的研究进展OA
Progress in Immune Checkpoint Inhibitors for Gestational Trophoblastic Tumors
妊娠滋养细胞肿瘤(gestational trophoblastic neoplasia,GTN)是由于妊娠滋养细胞异常发育和增殖导致,包括侵袭性葡萄胎/绒毛膜癌/胎盘部位滋养细胞肿瘤及上皮样滋养细胞肿瘤.尽管化疗是其主流且高效的初始治疗方案,完全治愈率可达 80%以上,但部分患者在治疗期间出现了耐药,使得治疗效果差,甚至无效/死亡,因此对耐药及复发患者的管理仍是临床挑战.近年来,免疫检查点抑制剂(immune checkpoint inhibitors,ICIs)在肿瘤治疗领域取得重大突破,其与化疗联合的策略为 GTN 的治疗提供了新思路.目前,关于 ICIs 联合化疗在 GTN 中应用的系统综述较少.本综述旨在系统总结 ICIs 与化疗等不同治疗方式联合治疗的研究进展,并进一步阐述其作用机制/疗效与安全性,同时,本文也将展望 ICIs 联合其他治疗模式以及未来的研究方向,为临床实践和后续研究提供参考.
Gestational trophoblastic neoplasia(GTN)is caused by the abnormal development and proliferation of gesta-tional trophoblastic cells,and includes invasive mole,choriocarcinoma,placental site trophoblastic tumor,and epithelioid trophoblastic tumor.Although chemotherapy is the mainstream and effective initial treatment,with a complete cure rate of o-ver 80%,some patients develop drug resistance during treatment,leading to poor outcomes,treatment failure,and even death.Therefore,the management of drug-resistant and relapsed patients remains a clinical challenge.In recent years,im-mune checkpoint inhibitors(ICIs)have achieved major breakthroughs in oncology.The strategy of combining ICIs with chemotherapy offers a new approach for the treatment of GTN.Currently,there are few systematic reviews on the application of ICIs combined with chemotherapy in GTN.This review aims to systematically summarize the research progress of ICIs com-bined with chemotherapy and other treatment modalities,and to further elaborate on their mechanisms of action,efficacy,and safety.Additionally,this article will discuss the prospects of ICIs combined with other treatment strategies and future re-search directions,with the goal of providing references for clinical practice and subsequent studies.
