子宫内膜癌分子分型已成为精准诊疗的新基石.2013 年美国癌症基因组图谱(The Cancer Genome Atlas,TCGA)计划提出 4 种分子分型:POLE 超突变型/微卫星不稳定型/低拷贝数型和高拷贝数型.ProMisE和 TransPORTEC 等简化分型策略使临床应用成为可能.分子分型对术前手术方式决策/术后辅助治疗制定/保留生育功能治疗选择及林奇综合征筛查均产生重要影响.然而,分子分型在临床应用中仍面临检测可及性/临床预后异质性和混合分子分型特征解读等挑战.未来需探索更简便准确的分型方法,建立无特定分子谱(no specific molecular profile,NSMP)型等异质性较大分型的二级分层体系,并通过前瞻性临床试验验证基于分型的治疗策略,最终构建动态/精准/可及的子宫内膜癌全程管理新范式.

Molecular classification of endometrial cancer has become a new cornerstone of precision diagnosis and treat-ment.In 2013,The Cancer Genome Atlas project proposed four molecular subtypes:POLE ultramutated,microsatellite in-stability,copy-number low,and copy-number high.Simplified classification strategies such as ProMisE and TransPORTEC have made clinical application possible.Molecular classification has significant implications for preoperative surgical deci-sion-making,postoperative adjuvant therapy planning,fertility-sparing treatment,and Lynch syndrome screening.Howev-er,clinical application still faces challenges in testing accessibility,clinical prognostic heterogeneity,and interpretation of mixed molecular features.Future research should explore more convenient and accurate classification methods,establish secondary stratification systems for highly heterogeneous subtypes such as no specific molecular profile(NSMP),and vali-date classification-based treatment strategies through prospective clinical trials to ultimately construct a dynamic,precise,accessible new paradigm for comprehensive endometrial cancer management.
