目的 了解不同标准下四川省南充市主城区6~18岁儿童青少年超重肥胖的流行现况及分布特征,为进一步开展防控工作提供参考依据.方法 采用分层整群随机抽样法,选取南充市主城区(顺庆、嘉陵、高坪)3505例中小学生进行体格检查和问卷调查,采用体质指数(BMI)和腰围身高比(WHtR)两种指标筛查儿童青少年超重肥胖.结果 BMI标准筛查的南充市主城区中小学生超重肥胖总检出率为35.09%(超重18.43%,肥胖16.66%),WHtR标准筛查的腹型超重肥胖总检出率为24.25%(腹型超重6.96%,腹型肥胖17.29%).两种标准筛查的各学段男生超重肥胖检出率均高于女生,差异有统计学意义(P<0.05).WHtR标准筛查的小学生腹型超重肥胖检出率高于初中生和高中生,顺庆区儿童青少年腹型超重肥胖检出率高于嘉陵区和高坪区,差异均有统计学意义(P<0.05).BMI标准筛查的不同学段、片区中小学生超重肥胖检出率差异无统计学意义.两种标准筛查儿童青少年超重肥胖的一致性程度一般(Kappa=0.666),其中小学男生的一致性较好(Kappa=0.777).结论 南充市主城区6~18岁儿童青少年超重肥胖和腹型超重肥胖状况均不容乐观,需高度重视并采取相应防控措施.

Objective To explore the prevalence and distribution characteristics of overweight and obesity among 6~18-year-old children and adolescents in the main urban area of Nanchong under different criteria,aiming to offer references for sub-sequent prevention and control efforts.Methods Through the application of the stratified cluster random sampling method,3505 primary and secondary school students from the main urban area of Nanchong(Shunqing,Jialing,and Gaoping)were selected for physical examinations and questionnaire surveys.Body mass index(BMI)and waist-to-height ratio(WHtR)were employed to screen for overweight and obesity.Results The overall detection rate of overweight and obesity screened by the BMI standard was 35.09%(18.43%for overweight and 16.66%for obesity),whereas that screened by the WHtR standard was 24.25%(6.96%for abdominal overweight and 17.29%for abdominal obesity).Under both criteria,the prevalence rates among boys in each grade were higher than those among girls with statistically significant differences(P<0.05).The detection rate of abdominal overweight and obesity among primary school students screened by the WHtR standard was higher than that among junior and senior high school students(P<0.05).The detection rate of abdominal overweight and obesity in Shunqing was higher than that in Jialing and Gaoping(P<0.05).There was no statistically significant difference in the detection rate of overweight and obesity among primary and secondary school students of different grades and districts screened by the BMI standard.The Kappa value comparing the BMI standard with the WHtR standard was 0.666,and for primary school boys,the Kappa value was 0.777,both indicating a high de-gree of consistency.Conclusion The situation regarding overweight/obesity and abdominal overweight/obesity among 6~18-year-old children and adolescents in the main urban area of Nanchong is not promising.It is imperative to attach great significance to this issue and implement corresponding prevention and control measures.
