老年2型糖尿病患者血清Apelin-13、sFRP4水平与骨质疏松症的关系研究OA
Relationships Between Serum Apelin-13,sFRP4 and Osteoporosis in Elderly Patients with Type 2 Diabetes Mellitus
目的 探究老年2型糖尿病(T2DM)患者血清脂肪细胞因子Apelin-13、分泌型卷曲相关蛋白4(sFRP4)水平与骨质疏松症的关系.方法 选取2021年7月至2024年12月我院收治的老年T2DM患者225例,根据是否发生骨质疏松症分为发生组(n=57)与未发生组(n=168),比较两组临床资料及血清Apelin-13、sFRP4水平,分析血清Apelin-13、sFRP4与骨密度(BMD)、骨代谢指标[Ⅰ型胶原羧基端肽β特殊序列(β-CTX)、25-羟基维生素D3[25(OH)D3]、骨碱性磷酸酶(BALP)、Ⅰ型前胶原氨基端延长肽(PINP)]的相关性,分析老年T2DM患者骨质疏松症相关因素及血清Apelin-13、sFRP4与老年T2DM患者骨质疏松症风险的非线性关系,并探讨血清Apelin-13、sFRP4交互作用对老年T2DM患者骨质疏松症风险的影响.结果 发生组女性占比及β-CTX、BALP、sFRP4水平高于未发生组,BMD及PINP、25(OH)D3、Apelin-13水平低于未发生组(P<0.05);老年T2DM发生骨质疏松症患者血清Apelin-13与β-CTX、BALP呈负相关,与BMD、PINP、25(OH)D3 呈正相关(P<0.05);血清 sFRP4 与 β-CTX、BALP 呈正相关,与 BMD、PINP、25(OH)D3 呈负相关(P<0.05);Logistic回归分析结果显示,调整其他混杂因素前后,血清Apelin-13、sFRP4均为老年T2DM患者骨质疏松症的影响因素(P<0.05);RCS分析显示,血清Apelin-13、sFRP4水平与老年T2DM患者骨质疏松症风险呈非线性关系(P<0.05);交互作用分析显示,血清Apelin-13、sFRP4对老年T2DM患者骨质疏松症风险存在拮抗作用(P<0.05).结论 老年T2DM患者血清Apelin-13水平下降、sFRP4水平升高与骨质疏松症发生有关,且二者之间可能存在拮抗作用,共同破坏骨形成与吸收动态平衡,增加骨质疏松症发生风险.
Objective To explore the relationship between serum adipocytokine Apelin-13 and secretory frizzled-related protein 4(sFRP4)levels and osteoporosis in elderly patients with type 2 diabetes mellitus(T2DM).Methods From July 2021 to December 2024,total 225 elderly T2DM patients treated in our hospital were selected.Based on whether osteoporosis occurred,pa-tients were divided into an occurrence group(n=57)and a non-occurrence group(n=168).Clinical data,serum Apelin-13 and sFRP4 levels were compared.The correlation between serum Apelin-13,sFRP4 levels and bone mineral density(BMD),as well as bone metabolic indicators including type Ⅰ collagen carboxy terminal peptide beta specific sequence β-CTX,25-hydroxyvitamin D3[25(OH)D3],bone alkaline phosphatase(BALP),and type Ⅰ procollagen amino terminal extension peptide(PINP),was ana-lyzed.Factors related to osteoporosis in elderly T2DM patients were investigated,and the non-linear relationship between serum Apelin-13,sFRP4 levels and the risk of osteoporosis was explored.Additionally,the impact of the interaction between serum Ape-lin-13 and sFRP4 levels on the risk of osteoporosis in elderly T2DM patients was studied.Results In the occurrence group,the proportion of women,and β-CTX,BALP,and sFRP4 were higher,while BMD,PINP,25(OH)D3,and Apelin-13 were lower com-pared with the non-occurrence group(P<0.05).In elderly T2DM patients with osteoporosis,serum Apelin-13 was negatively cor-related with β-CTX and BALP,and positively correlated with BMD,PINP,and 25(OH)D3(P<0.05).Serum sFRP4 was posi-tively correlated with β-CTX and BALP,and negatively correlated with BMD,PINP,and 25(OH)D3(P<0.05).Logistic regres-sion analysis indicated that both serum Apelin-13 and sFRP4 levels were influencing factors of osteoporosis in elderly T2DM pa-tients before and after adjusting for other confounding factors(P<0.05).Restricted cubic spline(RCS)analysis showed a non-linear relationship between serum Apelin-13 and sFRP4 levels and the risk of osteoporosis in elderly T2DM patients(P<0.05).Interaction analysis revealed an antagonistic effect between serum Apelin-13 and sFRP4 levels on the risk of osteoporosis in elderly T2DM patients(P<0.05).Conclusion In elderly patients with T2DM,a decrease in serum Apelin-13 and an increase in sFRP4 are associated with the occurrence of osteoporosis.There may be an antagonistic effect,which jointly disrupts the dynamic balance of bone formation and absorption,thereby increasing the risk of osteoporosis.
