心脏磁共振延迟增强成像对肥厚型心肌病患者心肌纤维化的随访评估OA
Follow-Up Assessment of Myocardial Fibrosis in Patients with Hypertrophic Cardiomyopathy by Cardiac Magnetic Resonance Delayed Enhancement Imaging
目的 探讨基于心脏磁共振成像(CMR)的延迟强化(LGE)技术在肥厚型心肌病(HCM)患者在随访评价左室心肌纤维化的价值.方法 回顾性收集经两次心脏磁共振扫描(CMR1-CMR2)的HCM患者21例,获得心脏电影及LGE图像,随后分析得到左右心室功能及LGE参数,并采集患者相关临床资料.采用配对t检验来比较左右心室功能参数.结果 在进行首次CMR扫描时,有10例患者存在LGE.CMR1与CMR2之间的间隔为(24.86±15.25)月.在随访复查过程中,HCM患者的LGE发生率逐渐增加,但差异无统计学意义(P>0.05).但是,HCM患者的LGE总量明显增多[0.80%(0%,3.60%)vs.5.3%(1.35%,8.60%),P<0.001].随着疾病的进展,HCM患者左室舒张末期最大厚度明显增加[17.00 mm(15.00 mm,19.00 mm)vs.18.00 mm(16.00 mm,19.00 mm),P=0.027].但是,CMR1 与 CMR2 的左右心室射血分数、左室质量及左室重构指数差异无统计学意义(均P>0.05).此外,左心室舒张末期最大厚度增量(r=0.550,P=0.014)及左心室质量增量(r=0.516,P=0.028)与HCM患者的LGE增加量呈正相关.结论 HCM患者的心肌纤维化范围随着病程进一步增加,且心肌纤维化增量与左心室质量及舒张末期最大厚度的增加相关.
Objective To explore the value of late gadolinium enhancement(LGE)technology based on cardiac magnetic resonance imaging(CMR)in assessing left ventricular myocardial fibrosis in patients with hypertrophic cardiomyopathy(HCM)during follow-up.Methods A retrospective study was conducted on 21 patients with HCM who underwent two cardiac magnetic resonance examinations(CMR1-CMR2).All patients completed CMR examinations,from which cine and LGE images were ac-quired.Left and right ventricular function and LGE parameters were analyzed,and relevant clinical data of the patients were collect-ed.Paired t-test was used to compare left and right ventricular functional parameters.Results At the first CMR scan(CMR1),10 HCM patients had LGE.The interval between CMR1 and CMR2 was(24.86±15.25)months.During follow-up,the incidence of LGE in HCM patients gradually increased,but the difference was not statistically significant(P>0.05).However,the total amount of LGE in HCM patients increased significantly[0.80%(0%,3.60%)vs.5.3%(1.35%,8.60%),P<0.001].With disease progression,maximum left ventricular end-diastolic thickness increased significantly in HCM patients[17.00 mum(15.00 mm,19.00 mm)vs.18.00 mm(16.00 mm,19.00 mm),P=0.027].However,there was no statistically significant difference in left and right ventricular ejection fraction,left ventricular mass and left ventricular remodel index between CMR1 and CMR2(all P>0.05).In addition,the maximum increase in left ventricular end-diastolic thickness(r=0.550,P=0.014)and left ventricular mass(r=0.516,P=0.028)was positively correlated with the increase in LGE in HCM patients.Conclusion The myocardial fi-brosis of HCM patients increases with disease progression,and the increase correlates with the increase of left ventricular mass and end-diastolic peak thickness.
