癌症患者家庭主要照顾者沟通模式与照顾负担及心理脱离的关系研究OA

Objective To explore the relationship between different family communication patterns and psychological detachment and the mediating effect of caregiving burden,and to provide reference for improving the level of psychological detachment.Methods A total of 388 primary family caregivers of cancer patients were selected by convenience sampling and investigated with the Family Caregiver Burden Scale for Cancer patients,Family Caregiver Communication Model Scale and Psychological Detachment scale.The direct effect of different family communication models on psychological detachment and the mediating effect of caregiving burden were tested by the mediating effect analysis of multi-category independent variables.Results The total score of psychological detachment of primary family caregivers of cancer patients was 7.63±3.35.Mediation effect analysis showed that compared to cooperative communi-cation mode,the commitment,dominant and lonely communication modes had a relatively direct effect on psychological detachment(β=-0.289,-0.665,-0.685,all P<0.001),and the relatively indirect effect of caregiving burden was significant(β=-0.207,-0.935,-0.897,all P<0.05),and care burden played a partial mediating role.Conclusions The psychological detachment of primary family caregivers of cancer patients is at a lower middle level.The main family caregivers of cancer patients with different communication modes have different performances in care burden and psychological detachment.Caregiving burden plays a partial mediating role between family communication mode and psychological detachment.Medical staff should provide personalized communication guid-ance for caregivers according to diverse family communication modes,cultivate their awareness of active communication,improve their willingness to active communication,and reduce caregiving burden,so as to promote the improvement of psychological detachment.
