目的 探讨多囊卵巢综合征(polycystic ovary syndrome,PCOS)患者发生高同型半胱氨酸血症(hyperhomocysteinemia,HHCY)的危险因素分析及预测模型构建.方法 回顾性分析2022年1月至2025年1月南阳市中心医院306例PCOS患者临床资料.依据同型半胱氨酸(Hcy)是否≥15 µmol/L,将其分为HHCY组(n=97)及非HHCY组(n=209),收集患者一般资料,采用Logistic回归分析影响PCOS患者发生HHCY的危险因素,并依据危险因素构建综合指数;绘制受试者工作特征曲线(ROC)分析PCOS患者发生HHCY预测价值.结果 年龄、体重指数(body mass index,BMI)、妊娠糖尿病、睾酮(testosterone,T)、胆红素(bilirubin,TBil)、直接胆红素(direct bilirubin,DBil)、空腹胰岛素(fasting insulin,FINS)及胰岛素抵抗指数(homeostasis model assessment of insulin resistance,HOMA-IR)是PCOS患者发生HHCY的影响因素(P<0.05).多因素Logistic回归分析结果显示,BMI高、T水平高及HOMA-IR值高是影响PCOS患者发生HHCY的独立危险因素(P<0.05);DBil水平低是影响PCOS患者发生HHCY的独立保护因素(P<0.05).ROC曲线分析结果显示,BMI、T、DBil、HOMA-IR及综合指数预测PCOS患者发生HHCY的AUC值分别为0.892、0.766、0.743、0.762及0.992,综合指数预测价值最高.结论 高BMI、胰岛素抵抗、高T水平是PCOS患者发生HHCY的独立危险因素,而DBil为其独立保护因素,基于此构建的综合预测模型展现出较高的预测价值.

Objective We aimed to identify risk factors and develop a prediction model for hyperhomocysteinemia(HHcy)in patients with polycystic ovary syndrome(PCOS).Methods This study retrospectively reviewed 306 PCOS patients from Nanyang Cen-tral Hospital(Jan 2022-Jan 2025).Patients were categorized into HHCY(n=97)and non-HHCY(n=209)groups based on a homo-cysteine(Hcy)cutoff of≥15 µmol/L.After collecting baseline data,we employed logistic regression to identify risk factors for HHCY,built a composite index from these factors,and evaluated its predictive performance using ROC curve analysis.Results Age,body mass index(BMI),gestational diabetes,testosterone(T),bilirubin(TBil),direct bilirubin(DBil),fasting insulin(FINS),and insulin resistance(HOMA-IR)are influencing factors for the development of HHCY in patients with polycystic ovary syndrome(PCOS)(P<0.05).Multivariate Logistic regression analysis revealed that high BMI,high T level,and high HOMA-IR value were independent risk factors for the occurrence of HHCY in PCOS patients(P<0.05).A low DBil level was an independent protective factor against the occurrence of HHCY in PCOS patients(P<0.05).ROC curve analysis showed that the area under the curve(AUC)values for BMI,T,DBil,HOMA-IR,and the comprehensive index in predicting HHCY occurrence in PCOS patients were 0.892,0.766,0.743,0.762,and 0.992,respectively.The comprehensive index had the highest predictive value.Conclusion High BMI,insulin resistance,and elevated testosterone levels are independent risk factors for the development of HHCY in patients with PCOS,whereas DBil serves as an independent protective factor.The comprehensive prediction model constructed based on these factors demon-strates high predictive value.
