Causal association between melasma and chronotype based on Mendelian randomization and the perspective of Traditional Chinese Medicine

目的 依托孟德尔随机化(MR)方法,融入中医视角,探讨黄褐斑与时型间的因果关系.方法 首先获取芬兰基因库(FinnGen)黄褐斑数据与IEU开放GWAS时型(晨型/夜型)数据,据此筛选工具变量(Ⅳ);并通过MR多效性残差和离群值检验剔除干扰单核苷酸多态性(SNPs),保障Ⅳ质量.后续采用逆方差加权(IVW)、MR-Egger回归、加权中位数法进行MR分析,以MR-Egger截距检验评估Ⅳ多效性,Cochran's Q检验评估异质性,同时通过森林图、散点图实现结果可视化.结果 IVW法表明,时型与黄褐斑之间呈显著正相关(OR=0.007,95%CI:0.000,0.655,P=0.032).结论 黄褐斑与时型之间可能存在潜在因果关系,这不仅为黄褐斑的预防与治疗研究提供了新的切入点,更对深化临床实践具有重要意义.

Objective To investigate the causal association between melasma and chronotype,leveraging the Mendelian randomization(MR)method and incorporating perspectives from Traditional Chinese Medicine(TCM).Methods First,the chloasma data were obtained from the FinnGen Finnish biobank and the morningness/eveningness phenotype data from the IEU open GWAS,and instrumental variables(Ⅳ)were screened accordingly.Interfering single nucleotide polymorphisms(SNPs)were then removed using the Mendelian Randomization(MR)Pleiotropy Residual Sum and Outlier(MR-PRESSO)test to ensure the quality of IVs.Subsequently,inverse variance weighting(IVW),MR-Egger regression,and the weighted median method were employed for MR analysis.The MR-Egger intercept test was used to evaluateⅣ pleiotropy,and Cochran's Q test was applied to assess heterogeneity.Meanwhile,the results were visualized through forest plots and scatter plots.Results The IVW method showed a significantly positive correlation between chronotype and melasma(OR=0.007,95%CI:0.000-0.655,P=0.032).Conclusion This study confirms a potential causal relationship between melasma and chronotype.It not only provides a new avenue for research into the prevention and treatment of melasma but also holds significant importance for advancing clinical practice.
