膝骨关节炎患者血清软骨寡聚基质蛋白表达水平及临床意义OA
Expression level and clinical significance of serum cartilage oligomeric matrix protein in patients with knee osteoarthritis
目的 探讨膝骨关节炎(KOA)患者血清软骨寡聚基质蛋白(COMP)的表达水平及临床意义.方法 选择2020年5月—2022年5月收治的KOA患者100例为观察组,选择同期健康体检者100例为对照组.收集观察组和对照组性别、年龄、体质量指数等资料,采用酶联免疫吸附法检测两组患者血清COMP表达水平,同时采用Kellgren-Lawrence(K/L)分级对KOA患者病情程度进行分级.比较观察组和对照组血清COMP表达水平,采用Pearson和Spearman相关分析KOA患者血清COMP表达水平与基线资料、K/L分级的相关性.结果 观察组和对照组年龄、性别、体质量指数比较差异无统计学意义(P>0.05).观察组血清COMP表达水平高于对照组(P<0.01).血清COMP高表达组KOA患者、年龄和体质量指数均高于低表达组(P<0.01);COMP高表达组和低表达组性别比较差异无统计学意义(P>0.05).Pearson和Spearman相关性分析显示,观察组患者血清COMP表达水平与体质量指数及K/L分级呈显著正相关(r=0.55、0.62,P<0.001),与性别和年龄无显著相关性(r=0.01、0.09,P=0.912、0.359).单因素Logistic回归分析发现,血清COMP表达水平由低变为高,K/L分级重度风险增加5.22倍;多因素Logistic回归分析发现,在校正性别、年龄、体质量指数等因素后,血清COMP表达水平由低变为高,K/L分级重度风险增加5.98倍.结论 KOA患者血清COMP表达水平普遍增高,其表达水平可反映疾病的严重程度,可为临床诊断和治疗效果的评估提供科学依据.
Objective To investigate the expression of serum cartilage oligomeric matrix protein(COMP)in patients with knee osteoarthritis(KOA)and its clinical significance.Methods One hundred KOA patients admitted from May 2020 to May 2022 were selected as the observation group,and 100 healthy individuals undergoing physical examinations during the same period were selected as the control group.The gender,age,and body mass index(BMI)of the observation group and the control group were collected.The expression level of serum COMP in the two groups was detected by enzyme-linked immunosorbent assay(ELISA),and the severity of KOA in the patients was graded using the Kellgren-Lawrence(K/L)scale.The expression level of serum COMP was compared between the observation group and the control group,and the correlation of the expression level of serum COMP in KOA patients with baseline data and the K/L grade was analyzed using Pearson and Spearman correlation analyses.Results There was no significant difference in age,gender,and BMI between the observation group and the control group(P>0.05).The expression level of serum COMP in the observation group was higher than that in the control group(P<0.0I).The number of KOA patients,age,and BMI in the serum COMP high-expression group were all higher than those in the low-expression group(P<0.0 1);there was no significant difference in gender between the COMP high-expression group and the low-expression group(P>0.05).Pearson and Spearman correlation analyses showed that the expression level of serum COMP in the observation group was significantly and positively correlated with BMI and K/L grade(r=0.55,0.62,P<0.001),and there was no significant correlation with gender and age(r=0.01,0.09,P=0.912,0.359).Univariate logistic regression analysis found that when the expression level of serum COMP changed from low to high,the risk of severe K/L grade increased by 5.22 times.Multivariate logistic regression analysis found that after adjusting for gender,age,and BMI,when the expression level of serum COMP changed from low to high,the risk of severe K/L grade increased by 5.98 times.Conclusion The expression level of COMP in the serum of KOA patients is generally elevated.This expression level can reflect the severity of the disease and can provide a scientific basis for clinical diagnosis and the assessment of treatment efficacy.
