原发性中枢神经系统淋巴瘤2例报告并文献复习OA
Primary central nervous system lymphoma:report of 2 cases and literature review
目的 探讨原发性中枢神经系统淋巴瘤(PCNSL)的不典型症状及影像学特征,确立多模态影像联合早期病理活检的核心诊断价值,为优化该罕见肿瘤治疗提供理论与实践依据.方法 回顾性分析江西省宜春市人民医院神经外科2024 年9 月—2025 年3 月收治的2 例 PCNSL 病例,结合近5 年国内外最新研究进展,系统梳理 PCNSL 的诊疗现状与发展方向.结果 2 例患者临床表现均不典型,最终均通过活检病理证实为弥漫大 B 细胞淋巴瘤.1 例患者早期误诊,确诊后拒绝化疗,出院后半个月死亡;另1 例患者术后定期化疗,生存良好,但出院后1 个月因发生意外死亡.结论 原发性中枢神经系统淋巴瘤(PCNSL)临床表现隐匿,影像学异质性显著,常导致早期误诊率高.多模态影像有助于早期诊断,病理活检为确诊金标准.治疗中,HD-MTX 联合 CD20 靶向免疫治疗可提升疗效;未来需融合分子亚型,构建"影像早筛-分子确诊-靶向干预"动态联动体系,实现诊疗模式向循证精准化转型.
Objective To investigate the atypical clinical and imaging features of primary central nervous system lymphoma(PCNSL),establish the pivotal diagnostic value of multimodal imaging combined with early pathological biopsy,and provide theoretical and practical foundations for optimizing treatment strategies for this rare malignancy.Methods A retrospective analysis was conducted on two PCNSL cases admitted to the Department of Neurosurgery,Yichun People's Hospital,Jiangxi Province,from September 2024 to March 2025.Integrated with recent advances in global research over the past five years,this study systematically reviews current diagnostic and therapeutic approaches as well as future directions.Results Both cases presented atypical clinical manifestations and were pathologically confirmed as diffuse large B-cell lymphoma through biopsy.One patient was initially misdiagnosed,declined chemotherapy post-diagnosis,and succumbed two weeks after discharge.The other patient received regular postoperative chemotherapy with favorable outcomes,but experienced accidental death one month post-discharge.Conclusions Primary central nervous system lymphoma(PCNSL)demonstrates occult clinical presentations and significant imaging heterogeneity,contributing to high early misdiagnosis rates.Multimodal imaging facilitates preliminary diagnosis,while pathological biopsy remains the gold standard.Therapeutically,HD-MTX combined with CD20-targeted immunotherapy significantly improves treatment efficacy.Future protocols should integrate molecular subtyping to establish a dynamic interconnected system encompassing"imaging-based early screening,molecular confirmation,and targeted intervention,"thereby advancing evidence-based precision medicine in PCNSL management.
