目的 探讨脑胶质瘤患者血清细胞因子信号转导抑制因子 3(SOCS3)、趋化因子配体20(CCL20)水平与放疗敏感性的关系及其预测价值.方法 选取2023 年1 月—2025 年3 月期间沧州市人民医院就诊的122 例Ⅲ~Ⅳ级脑胶质瘤患者为研究对象,采用酶联免疫吸附(ELISA)法检测血清 SOCS3、CCL20水平.采用 Logistic 回归分析影响因素;绘制受试者工作特征(ROC)曲线分析预测价值;采用 Bootstrap 法内部验证联合预测模型校准度.结果 122 例脑胶质瘤患者中98 例放疗高敏感,24 例放疗低敏感,放疗低敏感率为19.67%.放疗低敏感组肿瘤病理分级Ⅳ级比例及治疗前、后血清 CCL20 水平高于放疗高敏感组,治疗前、后血清 SOCS3 低于放疗高敏感组(P<0.05).治疗前 SOCS3 是脑胶质瘤患者放疗低敏感的独立保护因素,治疗前 CCL20 是脑胶质瘤患者放疗低敏感的独立危险因素(P<0.05).治疗前血清 SOCS3、CCL20 水平单独及联合预测脑胶质瘤患者放疗低敏感的曲线下面积为0.839、0.833、0.943.校准曲线结果显示,实际概率和预测概率基本一致,联合预测模型校准度较高.结论 血清 SOCS3 低水平、CCL20 高水平可能与脑胶质瘤放疗低敏感的发生有关,二者联合可有效预测脑胶质瘤患者放疗低敏感.

Objective To explore the relationship and predictive value of serum suppressor of cytokine signaling 3(SOCS3)and C-C chemokine ligand 20(CCL20)levels with radiotherapy sensitivity in patients with glioma.Methods From January 2023 to March 2025,122 patients with glioma grades Ⅲ-Ⅳ admitted to Cangzhou People's Hospital were enrolled as the study subjects.Enzyme linked immunosorbent assay(ELISA)was used to detect serum SOCS3 and CCL20 levels.Logistic regression was used to explore the influencing factors.Receiver operating characteristic(ROC)curves were plotted to explore the predictive value.Bootstrap method was employed for internal validation of the calibration of the joint prediction model.Results Among 122 patients with glioma,98 were radiotherapy sensitive,and 24 were radiotherapy insensitivity,with a radiotherapy insensitivity rate of 19.67%.The radiotherapy insensitivity group had higher proportion of tumor pathological grades IV and levels of serum CCL20 before and after treatment than the radiotherapy high sensitive group,while the radiotherapy insensitivity group had lower levels of serum SOCS3 before and after treatment than the radiotherapy high sensitive group(P<0.05).Pre-treatment SOCS3 was an independent protective factor for radiotherapy insensitivity in glioma patients,while pre-treatment CCL20 was an independent risk factor for radiotherapy insensitivity in glioma patients(P<0.05).The area under curve values of predicting radiotherapy insensitivity in glioma patients based on serum SOCS3 and CCL20 levels alone and their combination before treatment were 0.839,0.833,and 0.943,respectively.The calibration curve results showed that the actual probability was basically consistent with the predicted probability,indicating that the joint prediction model had a high calibration degree.Conclusions Low level of serum SOCS3 and high level of CCL20 may be associated with the occurrence of radiotherapy insensitivity in gliomas.The combination of the two can effectively predict the radiotherapy insensitivity in patients with glioma.
