MRI参数Ktrans、Kep值与血清miR-487a-3p、miR-490-3p对脑胶质瘤分级的诊断价值OA
Diagnostic value of MRI parameters Ktrans value and Kep value,and serum miR-487a-3p and miR-490-3p for glioma grading in glioma patients
目的 探讨胶质瘤患者磁共振成像(MRI)参数容积转运常数(Ktrans)值和速率常数(Kep)值与血清微小 RNA(miR)-487a-3p、miR-490-3p 对脑胶质瘤分级的诊断价值.方法 选取2023 年1 月—2025 年1 月在漯河市中心医院治疗的脑胶质瘤患者80 例为研究对象,根据临床病理学恶性程度分为低级别组37 例(Ⅰ~Ⅱ级),高级别组43 例(Ⅲ~Ⅳ级),所有患者均采用 MRI 动态对比增加扫描获取 Ktrans 值和 Kep 值,qRT-PCR 法检测血清 miR-487a-3p、miR-490-3p 水平;Pearson 分析相关性;受试者工作特征(ROC)曲线分析Ktrans 值和 Kep 值与血清 miR-487a-3p、miR-490-3p 对脑胶质瘤分级诊断价值;采用 Bootstrap 法对联合诊断模型进行内部验证.结果 高级别组患者 Ktrans、Kep 值均高于低级别组患者,血清 miR-487a-3p、miR-490-3p 水平均低于低级别组患者(P<0.05);血清 miR-487a-3p、miR-490-3p 水平分别与 Ktrans 值(r=-0.370、-0.486)和 Kep 值(r=-0.319、-0.463)均呈显著负相关(P<0.05);Ktrans 值、Kep 值、miR-487a-3p、miR-490-3p 四者单独诊断脑胶质瘤分级的曲线下面积(AUC)为0.829、0.844、0.841、0.839,四者联合诊断的 AUC为0.960,四者联合优于各自单独诊断(Z四者联合-Ktrans值=3.175、Z四者联合-Kep值=2.840、Z四者联合-miR-487a-3p=2.734、Z四者联合-miR-490-3p=2.491,P<0.05);内部验证结果显示,Hosmer-Lemeshow 检验 χ2=10.053,P=0.261,校准曲线与理想曲线一致性较好.结论 胶质瘤患者 MRI 参数 Ktrans、Kep 值与血清 miR-487a-3p、miR-490-3p 联合检测可显著提高脑胶质瘤分级诊断效能.
Objective To discuss the diagnostic value of magnetic resonance imaging(MRI)parameters volume transport constant(Ktrans)and rate constant(Kep),and serum microRNA(miR)-487a-3p and miR-490-3p for glioma grading.Methods From January 2023 to January 2025,80 patients with glioma treated at Luohe Central Hospital were considered as subjects.Complying with the extent of malignancy in clinical pathology,the patients were grouped into a low-grade group of 37 cases(grades Ⅰ-Ⅱ)and a high-grade group of 43 cases(grades Ⅲ-Ⅳ).All patients were subjected to dynamic contrast-enhanced MRI scans to obtain Ktrans and Kepvalues.The qRT-PCR method was performed to detect serum miR-487a-3p and miR-490-3p.Pearson's method for analyzing correlations.Receiver operating characteristic(ROC)curve was used to explore the diagnostic value of MRI parameters Ktrans and Kep values,and serum miR-487a-3p and miR-490-3p in glioma grading.Internal validation of joint diagnostic models using bootstrap methodology.Results The high-grade group had clearly higher Ktrans and Kep values than the low-grade group,and clearly lower serum miR-487a-3p and miR-490-3p than the low-grade group(P<0.05).Serum miR-487a-3p and miR-490-3p were clearly negatively correlated with Ktrans value(r=-0.370,-0.486)and Kep value(r=-0.319,-0.463),respectively(P<0.05).The are under curve(AUC)of Ktrans value,Kep value,miR-487a-3p,and miR-490-3p in the diagnosis of glioma grading was 0.829,0.844,0.841,and 0.839,respectively.The AUC of the joint diagnosis of the four was 0.960,and the joint of the four was better than their individual diagnosis(Zjoint-Ktrans value=3.175,Zjoint-Kep value=2.840,Zjoint-miR-487a-3p=2.734,Zjoint-miR-490-3p=2.491,P<0.05).The internal validation results showed good agreement between the calibration curve and the ideal curve with the Hosmer-Lemeshow test χ2=10.053,P=0.261.Conclusions The joint of MRI parameters Ktrans and Kep values with serum miR-487a-3p and miR-490-3p can significantly enhance the diagnostic efficacy in grading brain gliomas.
