中老年人群牙周炎分期与人巨细胞病毒感染的相关性研究OA
Periodontitis staging and human cytomegalovirus infection in middle-aged and elderly population:A cross-sec-tional study
目的:探讨中老年人群龈沟液人巨细胞病毒(human cytomegalovirus,HCMV)感染与牙周炎分期的相关性.方法:纳入 154 例 55～75 岁受试者,进行牙周检查并分期,qPCR检测龈沟液HCMV载量,分层回归分析关联.结果:HCMV总体检出率 27.27%,检出率及载量随分期升高显著递增(健康组 8.93%→Ⅳ期 52.38%).校正混杂因素后,HCMV载量每增加 1 log10 copies/mL,分期升级风险增加 1.18 倍,附着丧失加深 1.14 mm,探诊深度增加0.69 mm,现存牙数减少 1.15 颗.结论:HCMV载量是中老年牙周炎严重程度的独立相关因素,病毒载量与牙周组织破坏程度呈显著剂量-效应关系.
Objective:To investigate the correlation between human cytomegalovirus(HCMV)infection in gingival crevicular fluid and periodontal disease staging in middle-aged and elderly populations.Methods:A total of 154 subjects aged 55～75 years underwent periodontal examination and staging.HCMV viral load in gingival crevicular fluid was detected via qPCR,with stratified regression analysis employed to assess associations.Results:Overall HCMV detection rate was 27.27%,with both detection rate and viral load significantly increasing across disease stages(Health group:8.93%→Stage IV:52.38%).After adjusting for confounding factors,each 1 log10 copies/mL increase in HCMV load was associated with a 1.18-fold higher risk of periodontal stage progression,1.14 mm greater attachment loss,0.69 mm deeper probing depth,and 1.15 fewer remaining teeth.Conclusion:HCMV viral load is an independent factor associated with periodontitis severity in middle-aged and elderly individuals,exhibiting a significant dose-response relationship with periodontal tissue destruction.
