口腔种植患者早期种植失败的影响因素及其列线图预测模型构建OA

目的:探讨口腔种植患者早期种植失败(implantation failure,IF)的影响因素,并构建预测种植失败的列线图模型.方法:回顾性分析2020 年4 月~2023 年4 月收治348 例口腔种植(种植体592 颗)患者资料,分成成功组和IF组.通过Logistic回归模型筛查早期IF相关因素.利用R软件构建预测早期IF列线图模型,受试者工作特征(receiver operating characteristic,ROC)曲线、校准曲线对模型的预测效果予以内部验证,采用决策曲线分析临床适用情况.结果:早期失败 49 颗种植体,失败率为 8.28%(49/592).IF组糖尿病比例、定期洁牙比例、骨质为Ⅳ类、种植体直径、表面处理技术与成功组比较有统计学差异(P<0.05).糖尿病(OR=2.152,95%CI:1.085~4.267)、未定期洁牙(OR=2.344,95%CI:1.163~4.721)、骨质为Ⅳ类(OR=5.100,95%CI:2.032~8.801)、种植体直径≤3.5 mm(OR=3.301,95%CI:1.620~6.724)、非大颗粒喷砂酸蚀(sandblasting and acid etching,SLA)表面处理技术表面处理(OR=3.286,95%CI:1.692~6.383)为口腔种植患者早期IF的危险因素.内部验证显示,预测模型曲线下面积(the area under the curve,AUC)为 0.837(95%CI:0.771~0.902),校准曲线预测早期IF的概率和实际概率接近,且拟合优度HL检验χ2=7.523,P=0.377.决策曲线显示,模型阈值概率处于 2%~58%时有着较高的净获益.结论:基于糖尿病、定期洁牙情况、骨质、种植体直径、表面处理技术构建的列线图可较好地预测口腔种植患者早期IF风险.

Objective:To explore the influencing factors of early implantation failure(IF)in oral implantation pa-tients and construct a nomogram model for predicting implantation failure.Methods:A retrospective analysis was conducted on the data of 348 patients with oral implants(592 implants)from April 2020 to April 2023,were assigned into successful group and IF group.Logistic regression model was used to screen for factors related to early IF.R software was applied to con-struct a nomogram model for predicting early IF.The receiver operating characteristic(ROC)curve and calibration curve were used to internally validate the predictive performance of the model.The decision curve was used to analyze clinical applicabil-ity.Results:There were 49 early implantation failure,with a failure rate of 8.28%(49/592).There were great differences in proportion of diabetes,proportion of regular tooth cleaning,sclerotin type IV,implant diameter,surface treatment technology between IF group and the successful group(P<0.05).Diabetes(OR=2.152,95%CI:1.085~4.267),irregular tooth clean-ing(OR=2.344,95%CI:1.163~4.721),sclerotin type IV(OR=5.100,95%CI:2.032~8.801),implant diameter≤3.5 mm(OR=3.301,95%CI:1.620~6.724),non-sandblasted with large grits and acid-etched(SLA)surface treatment(OR=3.286,95%CI:1.692~6.383)were risk factors for early IF in oral implantation.Internal validation showed that the area under the curve(AUC)of the prediction model was 0.837(95%CI:0.771~0.902).The probability for predicting early IF using the calibration curve was close to the actual probability,and the goodness of fit HL test showed that χ2=7.523,P=0.377.The decision curve showed that there was a higher net benefit when the threshold probability of the model was between 2%and 58%.Conclusion:The nomogram based on diabetes,regular dental cleaning,sclerotin,implant diameter,and surface treatment technology can better predict the risk of early IF in patients with oral implantation.
