MDT联合临床路径改善低龄腭裂整复术围术期医疗质量的效果分析OA
Effect analysis of multidisciplinary team combined with structured clinical pathway on improving perioperative quality in young children undergoing cleft palate repair
目的:分析多学科协作(multidisciplinary team,MDT)联合结构化临床路径对改善低龄腭裂整复术患儿围术期医疗质量的临床效果.方法:采用回顾性队列研究,将 2023 年 5 月～2024 年 5 月接受传统管理的 64 例患儿设为对照组,2024 年6 月～2025 年9 月接受MDT结合临床路径管理的64 例患儿设为干预组.比较两组患儿术前分离焦虑(PSAS)、苏醒期躁动(PAED)、术后早期疼痛(FLACC)、术后舒适度(COMFORT-B)、家属满意度及围术期严重不良事件(反流误吸、低氧血症、非计划二次插管、非计划二次手术等).结果:干预组PSAS评分[1(1,2)vs 4(4,4)]、PAED评分[2(1,2)vs 3(3,4)]、FLACC评分(3.1±0.8 vs 5.8±0.6)及术后 24 h COMFORT-B评分(7.5±1.6 vs 13.2±2.8)均显著低于对照组(均 P<0.001).干预组家属满意度显著高于对照组(95.2±2.3 vs 90.2±1.8,P<0.001).两组均未发生严重不良事件.结论:MDT联合结构化临床路径能有效减轻低龄腭裂整复术患儿的围术期应激反应,改善就医体验,具备临床推广应用价值.
Objective:To evaluate the clinical efficacy of multidisciplinary team(MDT)collaboration combined with structured clinical pathways in improving perioperative medical quality for young children undergoing cleft palate repair sur-gery.Methods:A retrospective cohort study was conducted.Sixty-four children managed conventionally from May 2023 to May 2024 formed the control group,while 64 children managed with MDT combined with clinical pathways from June 2024 to September 2025 formed the intervention group.Comparisons were made between groups for preoperative separation anxiety(parental separation anxiety scale,PSAS),emergence delirium(pediatric anesthesia emergence delirium scale,PAED),early postoperative pain(face,legs,activity,cry,consolability scale,FLACC),postoperative comfort(COMFORT-behavioral scale,COMFORT-B),and family satisfaction.Results:The intervention group demonstrated significantly lower PSAS scores[1(1,2)vs 4(4,4)],PAED score[2(1,2)vs 3(3,4)],FLACC score(3.1±0.8 vs 5.8±0.6),and 24-hour postoperative COM-FORT-B score(7.5±1.6 vs 13.2±2.8)were significantly lower than the control group(all P<0.001).Family satisfaction was significantly higher in the intervention group than in the control group(95.2±2.3 vs 90.2±1.8,P<0.001).No serious ad-verse events occurred in either group.Conclusion:MDT combined with a structured clinical pathway effectively reduces peri-operative stress responses in young children undergoing cleft palate repair,improves clinical outcomes and patient experience,and holds value for clinical implementation.
