养胃温脾汤联合四联疗法对Hp阳性消化性溃疡患者炎性因子和免疫功能的影响OA
Effects of Yangwei Wenpi Decoction Combined with Quadruple Therapy in the Treatment of Hp-positive Peptic Ulcer on Inflammatory Factors and Immune Function of Patients
目的:探究养胃温脾汤联合四联疗法治疗幽门螺杆菌(Hp)阳性消化性溃疡的效果.方法:纳入 2022 年 2 月—2024 年 2 月在湖口县中医医院就诊的Hp 阳性消化性溃疡患者 94 例,随机分为观察组和对照组,各 47 例.2 组均采用四联疗法,观察组加用养胃温脾汤.比较 2 组治疗前后血清炎性因子和免疫功能指标水平、中医证候积分、胃肠道黏膜组织病理改变评分、临床疗效、Hp清除率以及不良反应等.结果:治疗后,观察组血清白介素-17(IL-17)、肿瘤坏死因子-α(TNF-α)水平低于对照组,CD4+T 淋巴细胞水平、CD4+/CD8+高于对照组,CD8+T 淋巴细胞水平低于对照组,中医证候积分、胃肠道黏膜组织病理改变评分低于对照组,Hp 清除率及总有效率高于对照组,上述各项指标组间差异均有统计学意义(P<0.05).2 组胃肠道反应、皮疹等不良反应发生率比较无显著性差异(P>0.05).结论:养胃温脾汤联合四联疗法治疗 Hp 阳性消化性溃疡可改善患者炎症反应、免疫功能,促进胃肠道黏膜修复,疗效可靠且安全性高.
Objective:To explore the effect of Yangwei Wenpi Decoction combined with quadruple therapy in the treatment of Helicobacter pylori(Hp)-positive peptic ulcer.Methods:A total of 94 patients with Hp-positive peptic ulcer who visited the Traditional Chinese Medicine Hospital of Hukou County,Jiangxi Province from February 2022 to February 2024 were enrolled and rando mLy divided into an observation group and a control group,47 cases in each group.Both groups were treated with quadruple therapy.The observation group was given Yangwei Wenpi Decoction in addition.The serum inflammatory factors and immune function index levels were measured and compared between the two groups before and after treatment.The TCM syndrome scores and gastrointestinal mucosal pathological change scores of the two groups were compared.The efficacy and Hp clearance rate of the two groups were compared,and adverse reactions were recorded.Results:After treatment,the levels of serum interlukin(IL)-17 and tumor necrosis factor-α(TNF-α)in the Yangwei Wenpi Decoction group decreased more than those in the conventional treatment group(P<0.05);The increase in CD4+T lymphocytes and CD4+/CD8+and the decrease in CD8+T lymphocytes in the Wenpi Decoction group were greater than those in the conventional treatment group(P<0.05);after treatment,the TCM syndrome scores and gastrointestinal mucosa in the Yangwei Wenpi Decoction group were The decrease in histopathological change scores was greater than that in the conventional treatment group(P<0.05);the Hp clearance rate and total effective rate in the Yangwei Wenpi Decoction group were significantly higher than those in the conventional treatment group(P<0.05);gastrointestinal reactions and rashes in the two groups of patients There is no significant difference in the overall incidence of adverse reactions(P>0.05).Conclusion:Yangwei Wenpi Decoction combined with quadruple therapy can improve the inflammatory response and immune function of patients with Hp-positive peptic ulcer,promote gastrointestinal mucosal repair,and has reliable efficacy and high safety.
