目的 探讨血浆骨膜蛋白(POSTN)、核因子E2相关因子2(Nrf2)与咳嗽变异性哮喘(CVA)患儿气道炎症和哮喘控制的关系.方法 回顾性选取185例CVA患儿(CVA组)和同期185例健康体检儿童(对照组)作为研究对象,均采用酶联免疫吸附试验检测血浆POSTN、Nrf2水平,并检测气道炎症指标[嗜酸粒细胞(EOS)计数、呼出气一氧化氮(FeNO)].采用Spearman相关分析探讨CVA患儿血浆POSTN、Nrf2水平与病情程度和气道炎症的相关性,采用多因素Logistic回归分析及受试者工作特征(ROC)曲线分析CVA患儿哮喘控制不良的影响因素及血浆POSTN、Nrf2水平的预测效能.结果 与对照组比较,CVA组血浆POSTN水平和EOS计数、FeNO升高,血浆Nrf2水平降低,差异有统计学意义(P<0.05).轻度CVA、中度CVA、重度CVA患儿的血浆POSTN水平和EOS计数、FeNO依次升高,血浆Nrf2水平依次降低,差异有统计学意义(P<0.017).CVA患儿血浆POSTN水平与病情程度、EOS计数、FeNO均呈正相关(r=0.707、0.651、0.668,P<0.001),血浆Nrf2水平与病情程度、EOS计数、FeNO均呈负相关(r=-0.705、-0.603、-0.656,P<0.001).多因素Logistic回归分析显示,EOS计数升高、FeNO升高、POSTN升高是CVA患儿哮喘控制不良的独立危险因素,Nrf2升高是独立保护因素(P<0.05).ROC曲线显示,血浆POSTN、Nrf2联合预测CVA患儿哮喘控制不良的曲线下面积为0.912,预测效能优于单独预测;经1 000次Bootstrap法内部验证,联合预测的C指数为0.912(95%CI:0.906~0.918).结论 CVA患儿血浆POSTN水平升高和Nrf2水平降低,与病情加重、气道炎症加剧和哮喘控制不良相关.血浆POSTN、Nrf2或可成为哮喘控制不良的辅助预测指标,且二者联合具有更高的预测效能.

Objective To investigate the relationships of plasma periostin(POSTN),nuclear factor E2-related factor 2(Nrf2)with airway inflammation and asthma control in children with cough variant asthma(CVA).Methods A total of 185 children with CVA(CVA group)and 185 healthy children undergoing physical examinations during the same period(control group)were retrospective-ly selected as the study subjects.Enzyme-linked immunosorbent assay was used to detect the plasma levels of POSTN and Nrf2 in both groups,and airway inflammation indicators[eosinophil(EOS)count and fractional exhaled nitric oxide(FeNO)]were also measured.Spearman correlation analysis was employed to explore the correlations of plasma POSTN and Nrf2 levels with disease severity with airway inflammation in children with CVA.Multivariate logistic regression analysis and receiver operating characteristic(ROC)curve analysis were used to analyze the influencing factors of poor asthma control in children with CVA and the predictive efficacy of plasma POSTN and Nrf2 levels.Results Compared with the control group,the CVA group showed increased plasma POSTN levels,EOS counts,and FeNO levels,and decreased plasma Nrf2 levels,with statistically significant differ-ences(P<0.05).Among children with mild,moderate,and severe CVA,the plasma POSTN lev-els,EOS counts,and FeNO levels increased successively,while the plasma Nrf2 levels decreased successively,with statistically significant differences(P<0.017).In children with CVA,plasma POSTN levels were positively correlated with disease severity,EOS count,and FeNO(r=0.707,0.651,0.668,respectively;P<0.001),while plasma Nrf2 levels were negatively correlated with disease severity,EOS count,and FeNO(r=-0.705,-0.603,-0.656,respectively;P<0.001).Multivariate logistic regression analysis revealed that increased EOS count,increased FeNO,and increased POSTN were independent risk factors for poor asthma control in children with CVA,while increased Nrf2 was an independent protective factor(P<0.05).The ROC curve showed that the area under the curve for the combined prediction of poor asthma control in children with CVA by plasma POSTN and Nrf2 was 0.912,and the predictive efficacy was better than that of individual prediction.After internal validation using 1 000 Bootstrap resamplings,the C-index of the combined prediction was 0.912(95%CI,0.906 to 0.918).Conclusion Increased plasma POSTN levels and decreased plasma Nrf2 levels in children with CVA are associated with disease ex-acerbation,aggravated airway inflammation,and poor asthma control.Plasma POSTN and Nrf2 may serve as auxiliary predictive indicators for poor asthma control,and their combination has higher pre-dictive efficacy.
