代谢紊乱背景下心脑血管病治则由"活血化瘀"向"痰瘀同治"转化的探讨OA

Evolution of therapeutic strategies for cardio-cerebrovascular diseases from activating blood circulation to resolving both phlegm and blood stasis in the modern metabolic context

心脑血管疾病的中医病因病机随时代变迁不断演进,传统理论多以"血瘀"为核心,强调血脉阻滞,治以活血化瘀.随着现代生活方式与饮食结构的改变,代谢紊乱在心脑血管疾病发生发展中的作用日益凸显.中医学认为,代谢产物蓄积所表现出的粘滞、壅阻等特性,与"痰浊内生"的病理属性相契合,随着心脑血管疾病的病机由"单纯血瘀"逐步转向"痰瘀互结",治法亦相应由单一活血化瘀向"痰瘀同治"转变.本文在梳理历代理论演变的基础上,结合代谢紊乱背景,对心脑血管疾病"痰瘀互结"病机进行再认识,以期为相关治法治则的进一步研究提供理论参考.

The pathogenesis of cardiovascular and cerebrovascular diseases in traditional Chinese medicine(TCM)has been evolving constantly with the changes in lifestyles and disease patterns.The classical theory highlights blood stasis as their core pathogenesis and treats them by promoting blood circulation.With the changes in modern lifestyle and diet structure,metabolic disorders play an increasingly prominent role in the occurrence and development of these diseases.It is held in traditional Chinese medicine that the viscous and obstructive characteristics caused by the accumulation of metabolic products correspond to the pathological attribute of endogenous turbid phlegm.Accordingly,their core pathogenesis has shifted from simple blood stasis to intermingled phlegm and blood stasis,while their therapeutic strategies transmit as well from activating blood circulation alone to resolving both phlegm and blood stasis.Based on a review of the pathogenesis evolution of cardio-cerebrovascular diseases,combined with the background of metabolic disorders,this study re-examines their core pathogenesis of intermingled phlegm and blood stasis,in a bid to provide theoretical references for future research on their related treatment methods and principles.
