代谢组学在静脉血栓栓塞性疾病中的应用进展OA

静脉血栓栓塞(VTE)包括深静脉血栓形成(DVT)和肺栓塞(PE),病死率较高且发病率不断上升,其早期识别及诊断具有重要意义.代谢组学是对某一生物或细胞在特定生理时期的所有低分子量代谢产物同时进行定性和定量分析的一门新学科.近年来,众多研究采用代谢组学方法对VTE患者及动物模型的代谢物进行测定,得到一系列具有潜在早期诊断价值的生物标志物;根据对差异代谢物的代谢通路分析,推断VTE的发生发展可能与机体营养物质及能量代谢失衡、细胞信号传导、肉碱代谢、嘌呤代谢、肠道微生物代谢、炎症和氧化应激、遗传有关.此外,代谢组学对PE风险分层与预后、临床疗效评估及临床监测也具有一定的价值,但其临床应用还面临更多的挑战.本文综述了代谢组学在VTE的早期诊断、发病机制、风险分层和疗效评价等的相关研究进展.

Venous thromboembolism(VTE)includes deep vein thrombosis(DVT)and pulmonary embolism(PE)with high mortality and increasing morbidity.Early recognition and diagnosis of VTE is of great clinical significance.Metabolomics is a new discipline that performs qualitative and quantitative analysis of all low molecular weight metabolites of an organism or cell during a specific physiological period.In recent years,many studies have used metabolomics analysis to determine metabolites in patients and experimental animal models with VTE,and a series of biomarkers with potential early diagnosis value have been obtained.Moreover,according to the pathway enrichment analysis of differential metabolites,it is concluded that the pathophysiological processes of VTE may be related to the imbalance of nutrient and energy metabolism,cell signaling,carnitine metabolism,purine metabolism,intestinal microbial metabolism,inflammation and oxidative stress,and genetic correlation.In addition,metabolomics also has certain value in risk stratification and prognosis,clinical efficacy evaluation and clinical monitoring of PE.However,further studies are needed to implement metabolomics for clinical application of VTE.This review summarizes the progress of metabolomics research on early diagnosis,pathogenesis,risk stratification,and clinical efficacy evaluation of VTE.
