王耀献治疗糖尿病肾脏病用药规律研究OA
Professor Wang Yaoxian Medication Rule in Treatment of Diabetic Kidney Disease Based on Data Mining
目的:探讨王耀献教授治疗糖尿病肾脏病(diabetic kidney disease,DKD)临床用药规律.方法:以临床诊断 DKD 患者的病历资料为研究对象,筛选王耀献教授门诊患者病历,整理后逐一录入古今医案云平台中,利用统计分析、关联规则和聚类分析,对病例的一般资料、处方中药的性味归经、功效及常用药物组合进行分析,利用复杂网络分析预测处方用药规律,筛选清热益肾消癥方药.结果:本研究共纳入106 例患者,共计169 味中药.使用频率较高的中药为黄芪、桃仁、茯苓、水蛭等.药物功效排前3 位的分别为益气养阴类、活血化瘀类、清热类;四气以平性、温性、寒性居多;五味以甘味、辛味、苦味为主;归经以肝经、脾经、肾经常见.基于关联规则分析,生黄芪常与茯苓、党参、水蛭、海藻、鳖甲组成药对.层次聚类分析得到5 组常用药物组合,如生黄芪-桃仁-党参等.复杂网络分析生黄芪、桃仁、茯苓等15 味药物组成核心药物网络.结论:通过数据挖掘预测治疗 DKD 的核心药物主要为补脾益肾类、活血消癥类、清热养阴类和疏肝助阳类,针对不同病程阶段对症施治,注重通补结合、分期辨证、整体调节.
Objective:To explore Professor Wang Yaoxian's clinical medication rule in treating diabetic kidney disease(DKD).Meth-ods:The study focused on medical records of patients clinically diagnosed with DKD,screening DKD outpatients treated by Professor Wang Yaoxian.The records were organized and entered one by one into the Gujin Medical Case Cloud Platform.Statistical analysis,as-sociation rules,and cluster analysis were used to analyze the general patient information,the properties and meridian tropism of pre-scribed Chinese medicines,their efficacy,and common drug combinations.Complex network analysis was utilized to predict prescription patterns and to screen formulas and drugs for clearing heat,benefiting the kidneys,and resolving masses.Results:A total of 106 patients were included in this study,involving 169 Chinese medicines.The most frequently used Chinese medicines were Huangqi(Astragalus),Taoren(Peach kernel),Fuling(Poria),and Shuizzhi(Leech).The top three efficacies of the medicines were tonifying Qi and nouris-hing Yin,activating blood and resolving stasis,and clearing Heat.Regarding the Four Natures of the drugs,most of them were neutral,warm,or cold.Regarding the Five Flavors of the drugs,the main ones were sweet,pungent,and bitter.In terms of meridian tropism,the Liver,Spleen,and Kidney meridians were commonly involved.Based on association rule analysis,raw Astragalus is often paired with Po-ria,Dangshen(Codonopsis),Leech,Haizao(Seaweed),and Biejia(Turtle Shell).Hierarchical cluster analysis found five common drug combinations,such as Raw Astragalus-Peach Kernel-Codonopsis.Complex network analysis identified a core drug network con-sisting of 15 medicines including Raw Astragalus,Peach Kernel,and Poria.Conclusion:Data mining predicted that the core drugs for treating DKD mainly fall into categories of Spleen and Kidney tonifying,blood activation and mass elimination,Heat clearing and Yin nourishing as well as Liver soothing and Yang-assisting.Treatments are applied according to different disease stages,with emphasis on a combination of tonifying and dredging,staged syndrome differentiation,and overall regulation.
