从"水不利则病血"病机理论阐释慢性肾脏病并发血脉病变的内涵与外延OA
Connotation and Denotation of Blood Vessel Disorder in Chronic Kidney Disease Based on Pathogenesis Theory of"Shui Bu Li Ze Bing Xue"
心血管并发症为慢性肾脏病(chronic kidney disease,CKD)患者最常见的死亡原因,例如血管钙化、心力衰竭、心肌梗死等.从中医学角度出发,肾为"水脏",主水液代谢;而心主血脉,脉为血府.继发于 CKD 的心血管疾病为典型的由水液代谢不利引起的血脉运行不利的病症,故将该病机规律总结为"水不利则病血".在此过程中,"水不利"因素包含 CKD 原发病相关痰、饮、水、湿等病理因素的代谢障碍,"病血"则体现为血脉空虚及血脉瘀阻的病理结局.在此基础上,在病证同辨及水血同辨治则的指导下,可采用利水活血、除湿通络、祛痰逐瘀、填精充血等法治疗 CKD 相关血脉病变.
Cardiovascular complications are the most common cause of death in patients with chronic kidney disease(CKD),such as vascular calcification,heart failure,and myocardial infarction.From the perspective of traditional Chinese medicine,the Kidney is con-sidered as the"Water Zang",responsible for water metabolism,whereas the Heart governs the blood and vessels,with vessels being the repository of blood.Cardiovascular diseases secondary to CKD are a typical manifestation of impaired Water metabolism leading to dys-function in blood circulation,which can be summarized in the pathogenesis principle as"Shui(Water)Bu Li Ze Bing Xue(blood)".In this process,"Shui Bu Li"includes metabolic disturbances caused by pathologic factors related to CKD,such as Phlegm,Fluids,and Dampness,while"Bing Xue"is reflected in the pathological outcomes of blood vessel insufficiency and blood stasis.Based on this un-derstanding,guided by syndrome differentiation and disease differentiation as well as the principle of treating both Water and blood sim-ultaneously,methods such as promoting Water circulation to activate blood,removing Dampness to unblock meridians,dispelling Phlegm and removing stasis as well as replenishing Essence and blood can be applied to treat CKD-related vascular lesions.
