基于"分子-证候"双维度探讨不同亚型小细胞肺癌的中医精准辨治思路OA
Precise TCM Diagnosis and Treatment Approaches for Different Subtypes of Small Cell Lung Cancer in"Molecular-Syndrome"Dual Dimensions
小细胞肺癌(small cell lung cancer,SCLC)是一种侵袭性极强的神经内分泌肿瘤,目前小细胞肺癌最常见的亚型分类主要为基于转录因子所定义的 SCLC-A、SCLC-N、SCLC-P、SCLC-I 等.本文首次系统构建了 SCLC 分子分型与中医证型的映射关系,提出"分子-证候共演化"假说.设计"分型-辨证-复方"精准用药方案,将 SCLC 分子分型与中医辨证论治有机结合,可在一定程度上突破传统治疗的局限性,为个体化精准治疗提供新范式.同时也可在分子层面揭示中医证型的生物学基础(如 SCLC-I 亚型与"阴虚内热、正虚邪恋证"的免疫微环境关联),突破中医理论"抽象化"的瓶颈.虽然这一理论框架需要进一步的基础研究及临床验证,但其整合思维有望为肿瘤治疗提供一定的参考意义.未来需建立分子分型与中医证型的动态关联模型,开发标准化疗效评价体系,开展多中心临床研究,验证基于"分子-证候"双维度的"分型-辨证-复方"模式的可行性.
Small cell lung cancer(SCLC)is a highly aggressive neuroendocrine tumor.Currently,the most common subtype classifica-tion of SCLC is mainly based on transcription factors,defined as SCLC-A,SCLC-N,SCLC-P,SCLC-I,and other subtypes.This pa-per systematically constructs,for the first time,the mapping relationship between SCLC molecular subtypes and traditional Chinese med-icine(TCM)syndromes and proposes"molecular-syndrome co-evolution"hypothesis.A precise medication scheme of"syndrome recognition-syndrome differentiation-compound formula"is designed,organically combining SCLC molecular subtyping with TCM syndrome differentiation and treatment.This can,to some extent,overcome the limitations of traditional treatment and provide a new par-adigm for individualized precision therapy.At the same time,it can reveal the biological basis of TCM syndromes at the molecular level(for example,the association of the SCLC-I subtype with the immune microenvironment of"Yin deficiency with internal Heat,defi-ciency of the body's Zheng Qi with lingering Evil Factors"syndrome),breaking through the bottleneck of TCM theory's"abstract-ness".Although this theoretical framework requires further basic research and clinical validation,its integrative thinking is expected to provide certain reference value for tumor treatment.In the future,it is necessary to establish a dynamic correlation model between molec-ular subtypes and TCM syndromes,develop a standardized efficacy evaluation system,and conduct multicenter clinical studies to verify the feasibility of the"molecular-syndrome co-evolution"model based on the dual dimensions of"molecule-syndrome".
