目的:探讨祛腐生肌合剂调节 VEGF/PI3K/Akt 信号通路对大鼠慢性难愈性创面修复愈合的影响.方法:60 只 SD 大鼠按照1:5 随机分配至假手术组和模型组,模型组建模成功后再随机分配至创面组、中药组(灌胃1.35 g/kg 祛腐生肌合剂)、阳性药组(外用重组牛碱性成纤维细胞生长因子凝胶)、贝伐单抗(Bmab)组(腹腔注射5 mg/kg Bmab)、中药+Bmab 组(灌胃1.35 g/kg 祛腐生肌合剂、腹腔注射5 mg/kg Bmab),每组10 只.干预8d 后,测量创面面积并计算创面愈合率;采用 ELISA 法检测血清中 TNF-α、IL-1β 水平;采用免疫组化法检测创面组织中CD31 阳性面积百分比;采用qRT-PCR 法检测创面组织中转化生长因子β1(TGF-β1)、碱性成纤维生长因子(bFGF)mRNA 表达;采用 Western blot 法检测创面组织中 VEGF/PI3K/Akt 通路相关蛋白表达.结果:与假手术组相比,创面组创面愈合率,创面组织中 TGF-β1 及 bFGF mRNA 表达、CD31 阳性面积百分比、VEGF 蛋白表达、p-PI3K/PI3K、p-Akt/Akt 降低,血清中 TNF-α、IL-1β 水平升高(P<0.05).与创面组相比,中药组、阳性药组血清中 TNF-α、IL-1β水平降低,其他指标均升高(P<0.05).Bmab 削弱了祛腐生肌合剂对大鼠慢性难愈性创面修复愈合作用(P<0.05).结论:祛腐生肌合剂可通过激活 VEGF/PI3K/Akt 信号通路促进大鼠慢性难愈性创面修复愈合.

Aim:To investigate the effects of Qufu Shengji mixture on the repair and healing of chronic refractory wound in rats by regulating the VEGF/PI3K/Akt signaling pathway.Methods:Sixty SD rats were randomly assigned to the sham operation group and the model group at a ratio of 1:5.After successful modeling,rats were randomly divided into the wound group,the traditional Chinese medicine group(1.35 g/kg Qufu Shengji mixture by gavage),the positive drug group(topical application of recombinant bovine basic fibroblast growth factor gel),the bevacizumab(Bmab)group(intraperitone-al injection of 5 mg/kg Bmab),and the traditional Chinese medicine+Bmab group(1.35 g/kg Qufu Shengji mixture by ga-vage,intraperitoneal injection of 5 mg/kg Bmab),with 10 rats in each group.After 8 days intervention,the wound healing rate of the rats was detected.The levels of TNF-α and IL-1β in the serum were determined by ELISA.The CD31 positive ar-ea percentage in the wound tissue was detected by immunohistochemistry.The mRNA expressions of transforming growth factor β1(TGF-β1)and basic fibroblast growth factor(bFGF)in the wound tissue were detected by qRT-PCR.The expres-sions of VEGF/PI3K/Akt pathway-related proteins in the wound tissue was detected by Western blot.Results:Compared with the sham operation group,the wound healing rate,the mRNA expression levels of TGF-β1 and bFGF,the CD31 positive area percentage,the protein expression level of VEGF,and the ratios of p-PI3K/PI3K and p-Akt/Akt in the wound tissue of the wound group decreased,while the levels of TNF-α and IL-1β in the serum increased(P<0.05).Compared with the wound group,the levels of TNF-α and IL-1β in the serum of the traditional Chinese medicine group and the positive drug group decreased,while the other indexes increased(P<0.05).Bmab weakened the repair and healing effects of the Qufu Shengji mixture on chronic refractory wounds in rats(P<0.05).Conclusion:Qufu Shengji mixture could promote the re-pair and healing of chronic refractory wound in rats by activating VEGF/PI3K/Akt signaling pathway.
