基于核因子κB信号通路探究舒脉通络汤联合贝前列素钠治疗下肢动脉硬化闭塞症的疗效与作用机制OA

Study on the efficacy and mechanism of Shumai Tongluo Decoction combined with beraprost sodium for lower extremity arteriosclerosis obliterans based on the NF-κB signaling pathway

目的 观察舒脉通络汤治疗下肢动脉硬化闭塞症(ASO)的临床疗效,并基于核因子 κB(NF-κB)信号通路探讨其可能的作用机制.方法 将 86 例 ASO 患者按照随机数字表法分为对照组和治疗组,各 43 例.对照组予西医常规治疗,治疗组在对照组基础上加用舒脉通络汤治疗.2 组均治疗 21 d 后统计疗效,并比较 2 组治疗前后临床症状评分及血清肿瘤坏死因子(TNF-α)、血管内皮因子(VEGF)、白细胞介素 6(IL-6)、核因子 κB(NF-κB)水平变化情况.结果 与本组治疗前比较,2 组治疗后临床症状评分水平均降低(P<0.05),且治疗组治疗后临床症状评分低于对照组(P<0.05).与本组治疗前比较,2 组治疗后血清 TNF-α、NF-κB、VEGF、IL-6 水平均降低(P<0.05),且治疗组治疗后血清TNF-α、NF-κB、VEGF 水平均低于对照组(P<0.05).治疗组总有效率 88.37%(40/43),对照组总有效率 67.44%(38/43),治疗组总有效率高于对照组(P<0.05).结论 舒脉通络汤可能通过抑制/NF-κB 信号通路降低炎症反应,改善血管内皮功能,从而缓解 ASO 的临床症状延缓疾病进展.

Objective To explore the clinical efficacy of Shumai Tongluo Decoction for lower extremity arteriosclerosis obliterans(ASO),and its possible mechanism based on the nuclear factor κB(NF-κB)signaling pathway.Methods Eighty-six patients with ASO were randomized 1:1 to receive routine Western medicine(RWM)(control group)or RWM+Shumai Tongluo Decoction(treatment group)for 21 days,the aim was to compare the clinical symptom scores,serum tumor necrosis factor-α(TNF-α),vascular endothelial growth factor(VEGF),interleukin-6(IL-6),and NF-κB levels,and assess the curative effect.Results After treatment,the clinical symptom scores in the both groups were significantly lower than those before treatment(P<0.05),the decrease in the treatment group was more common than the control group(P<0.05).The serum TNF-α,NF-κB,VEGF,and IL-6 levels were significantly lower in both groups after treatment(P<0.05),and the serum TNF-α,NF-κB,and VEGF levels in the treatment group were significantly lower than those in the control group(P<0.05).The overall effective rate in the treatment group was significantly higher than that in the control group(88.37%[40/43]vs 67.44%[38/43],P<0.05).Conclusion Shumai Tongluo Decoction may reduce inflammatory response and improve vascular endothelial function by inhibiting the NF-κB signaling pathway,thereby alleviating the clinical symptoms and delaying disease progression of patients with ASO.
