目的 观察基于"任督通调"理论中药冰刺激联合中医适宜技术干预治疗卒中后吞咽障碍的临床效果.方法 选取2022 年1 月至2023 年12 月收治90 例卒中后吞咽障碍患者为研究对象,按照抽签法随机分为研究组和对照组,各 45 例.对照组予常规康复干预方案,研究组在对照组基础上加用基于"任督通调"理论中药冰刺激联合中医适宜技术干预方案.2 组均干预 4 个疗程后,比较 2 组治疗前后吞咽功能[分别采用洼田饮水试验、吞咽功能评定量表(SSA)、功能性经口摄食量表(FIOS)]、表面肌电图(包括吞咽时间、最大波幅)、营养状况[包括血清前白蛋白(ALB)、总蛋白(TP)、白蛋白(PA)]、生活质量综合评定问卷(GQOLI-74)评分(包括心理、躯体、社会功能、物质生活状态 4 个维度)变化情况.结果 与本组治疗前比较,2 组治疗后洼田饮水试验评分、SSA 评分均降低(P<0.05),FIOS 评分均升高(P<0.05),且研究组治疗后洼田饮水试验评分、SSA 评分均低于对照组(P<0.05),FIOS 评分高于对照组(P<0.05).与本组治疗前比较,2 组治疗后肌电图指标最大波幅水平均升高(P<0.05),吞咽时间均缩短(P<0.05),且研究组治疗后最大波幅水平高于对照组(P<0.05),吞咽时间短于对照组(P<0.05).与本组治疗前比较,2 组治疗后血清 ALB、PA、TP 水平均升高(P<0.05),且研究组治疗后血清 ALB、PA、TP 水平均高于对照组(P<0.05).与本组治疗前比较,2 组治疗后GQOLI-74 心理、躯体、物质生活状态、社会功能评分均升高(P<0.05),且研究组治疗后 GQOLI-74 心理、躯体、物质生活状态、社会功能评分均高于对照组(P<0.05).结论 基于"任督通调"理论中药冰刺激联合中医适宜技术干预治疗卒中后吞咽障碍患者疗效满意,可提高患者吞咽功能,经口摄食能力,改善肌电图指标,提高营养水平及生活质量.

Objective To evaluate the clinical efficacy of herbal ice stimulation guided by the theory of harmonizing the Ren and Du meridians combined with appropriate traditional Chinese medicine(TCM)techniques in patients with post-stroke dysphagia.Methods Ninety patients with post-stroke dysphagia treated between January 2022 and December 2023 were enrolled and randomly assigned by drawing lots to a study group or a control group,with 45 patients in each group.The control group received routine rehabilitation interventions.The study group received the same routine care plus adjunctive herbal ice stimulation based on the theory of harmonizing the Ren and Du meridians together with appropriate TCM techniques.After four treatment courses,the two groups were compared on swallowing function,measured by the water swallow test(WST),the Swallowing Function Assessment Scale(SSA),and the Functional Oral Intake Scale(FIOS);surface electromyography parameters,including swallowing time and maximum amplitude;nutritional status,represented by serum prealbumin(ALB),total protein(TP),and albumin(PA);and quality of life assessed by the General Quality of Life Inventory-74(GQOLI-74)across psychological,physical,social function,and material life status domains.Results Relative to baseline,both groups showed significant improvement in swallowing function after treatment:WST and SSA scores decreased(P<0.05)and FIOS scores increased(P<0.05).Post-treatment,the study group had significantly lower WST and SSA scores and higher FIOS scores than the control group(P<0.05).Surface electromyography showed increased maximum amplitude and shortened swallowing time in both groups compared with baseline(P<0.05).The study group demonstrated greater increases in maximum amplitude and shorter swallowing time than the control group after treatment(P<0.05).Nutritional markers ALB,PA,and TP rose significantly in both groups after treatment(P<0.05),with higher post-treatment ALB,PA,and TP levels in the study group compared with the control group(P<0.05).Quality of life scores on the GQOLI-74 in psychological,physical,material life status,and social function domains improved in both groups after treatment(P<0.05),and improvements were greater in the study group than in the control group(P<0.05).Conclusion The combined intervention of herbal ice stimulation based on the theory of harmonizing the Ren and Du meridians and appropriate TCM techniques provided satisfactory benefits for patients post-stroke dysphagia.The intervention improved swallowing function and oral intake capacity,enhanced electromyographic indicators,elevated nutritional markers,and improved overall quality of life.
