血清25-羟维生素D载脂蛋白A1与类风湿关节炎患者病理特征关系研究OA
A Study on the Relationship between Serum 25-Hydroxyvitamin D Apolipoprotein A1 and Pathological Characteristics in Patients with Rheumatoid Arthritis
目的:探究血清 25-羟维生素 D[25(OH)D]、载脂蛋白 A1(ApoA1)与类风湿关节炎(RA)患者炎症、骨代谢指标及并发骨质疏松(OP)的关系.方法:选取2022 年1 月至 2024 年 3 月我院收治的 RA 患者164 例为 RA 组,同期来我院体检的健康志愿者 120 例为对照组,酶联免疫吸附法检测血清25(OH)D、ApoA1 及炎症因子(IL-8、TNF-α、IL-17)、骨代谢标志物(β-CTX、B-ALP、PINP)水平,采用 Pearson 检验分析血清25(OH)D、ApoA1 与炎症、骨代谢指标间的相关性.统计 RA 患者入院时并发骨质疏松情况,比较非骨质疏松组和骨质疏松组临床资料及血清 25(OH)D、ApoA1 水平,采用Logistic 回归模型分析 RA 患者并发骨质疏松的影响因素.结果:与对照组比较,RA 组血清 ApoA1、白细胞介素-17(IL-17)、白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)、Ⅰ型胶原羧基端肽 β 特殊序列(β-CTX)显著升高(P<0.05),血清 25(OH)D、骨碱性磷酸酶(B-ALP)、Ⅰ型前胶原氨基端前肽(PINP)显著降低(P<0.05).相关性分析提示,血清 ApoA1 与 IL-8、TNF-α、IL-17、β-CTX 呈正相关(r=0.502,0.456,0.489,0.511,P<0.05),与 B-ALP、PINP 呈负相关(r=-0.440,-0.476,P<0.05);血清25(OH)D 与 IL-8、TNF-α、IL-17、β-CTX 呈负相关(r=-0.436,-0.477,-0.526,-0.483,P<0.05),与B-ALP、PINP 呈正相关(r=0.514,0.455,P<0.05).164 例 RA 患者中并发骨质疏松 65 例(占 39.63%).与非骨质疏松组比较,骨质疏松组年龄、RA 病程、ApoA1 明显升高(P<0.05),25(OH)D 显著降低(P<0.05),糖皮质激素持续时间、性别、吸烟史、饮酒史、骨折病史、疾病活动度没有统计学差异(P>0.05).多因素 Logistic 回归分析显示,年龄大、RA 病程长、25(OH)D 低、ApoA1 高是 RA 患者并发骨质疏松较差的危险因素(P<0.05).结论:血清 25(OH)D 低水平、ApoA1 高水平与类风湿关节炎患者炎症因子异常升高、骨代谢紊乱及并发骨质疏松有关,有望作为评估 RA 并发骨质疏松的潜在指标.
Objective:To investigate the relationship between serum 25-hydroxyvitamin D[25(OH)D],apolipoprotein A1(ApoA1),inflammation,bone metabolism indicators,and concurrent osteoporosis(OP)in patients with rheumatoid arthritis(RA).Methods:From January 2022 to March 2024,164 RA patients ad-mitted to our hospital were selected as the RA group,and 120 healthy controls who came for physical examina-tion at the same time were selected as the control group.Serum levels of 25(OH)D,ApoA1,inflammatory fac-tors(IL-8,TNF-α,IL-17),and bone metabolism markers(β-CTX,B-ALP,PINP)were detected by en-zyme-linked immunosorbent assay.The Pearson test was used to analyze the correlations of serum 25(OH)D and ApoA1 with indicators of inflammation and bone metabolism.The occurrence of osteoporosis in RA patients at admission was statistically analyzed.The clinical data and serum 25(OH)D and ApoA1 levels of the non-osteoporosis group and the osteoporosis group were compared.A logistic regression model was used to analyze the influencing factors of osteoporosis in RA patients.Results:Compared with the control group,the serum lev-els of ApoA1,interleukin-17(IL-17),interleukin-8(IL-8),tumor necrosis factor-α(TNF-α),and type I collagen carboxy-terminal peptide β-specific sequence(β-CTX)in the RA group were significantly in-creased(P<0.05),while the serum levels of 25(OH)D,bone alkaline phosphatase(B-ALP),and type I procollagen amino-terminal propeptide(PINP)were significantly decreased(P<0.05).Correlation analysis indicated that serum ApoA1 was positively correlated with IL-8,TNF-α,IL-17,and β-CTX(r=0.502,0.456,0.489,0.511,P<0.05),and negatively correlated with B-ALP and PINP(r=-0.440,-0.476,P<0.05);serum 25(OH)D was negatively correlated with IL-8,TNF-α,IL-17,and β-CTX(r=-0.436,-0.477,-0.526,-0.483,P<0.05),and positively correlated with B-ALP and PINP(r=0.514,0.455,P<0.05).Among 164 RA patients,65 cases(39.63%)had osteoporosis.Compared with the non-osteoporosis group,age,RA duration and ApoA1 in the osteoporosis group were significantly increased(P<0.05),25(OH)D was significantly decreased(P<0.05).There were no statistically significant differences in the duration of glucocorticoid use,gender,smoking history,drinking history,fracture history,and disease activity(P>0.05).Multivariate Logistic regression analysis showed that older age,longer RA duration,low 25(OH)D,and high ApoA1 were risk factors for poor osteoporosis in RA patients(P<0.05).Conclusion:Low serum 25(OH)D levels and high ApoA1 levels are associated with abnormally elevated inflammatory factors,bone metabolism disorders and the development of osteoporosis in patients with RA;they can possibly be used as potential markers for assessing osteoporosis associated with RA.
