[目的]探讨补中益气汤改善痔病的分子机制.[方法](1)动物实验:采用外敷冰醋酸法构建痔病模型.将20只SD大鼠随机分成对照组,痔病组及补中益气汤低、中、高剂量组,每组4只.连续干预7 d.苏木精-伊红(HE)染色法观察肛周组织病理变化,免疫组织化学法检测肛周组织Fibulin 5、α-SMA表达.(2)细胞实验:①收集Ⅲ度脱垂性内痔患者的痔病组织样本,提取原代成纤维细胞.用10%、20%、30%不同浓度的补中益气汤含药血清培养基培养,采用细胞计数试剂盒8(CCK-8)法和细胞划痕实验分别评估补中益气汤含药血清对原代成纤维细胞增殖和迁移的影响.②设置对照组、痔病组、补中益气汤组,采用Western Blot法、实时定量聚合酶链反应(RT-qPCR)法分别检测Fibulin 5、LTBP-4、α-SMA、CollagenⅠ蛋白和基因的表达.③采用siRNA技术敲低成纤维细胞中Fibulin 5,设置对照组、痔病组、补中益气汤组和补中益气汤+siRNA组,采用Western Blot法、RT-qPCR法分别检测Fibulin 5、LTBP-4、α-SMA、CollagenⅠ蛋白和基因的表达.[结果](1)动物实验结果:补中益气汤能改善痔病大鼠肛周组织的病变程度;痔病组大鼠肛周组织Fibulin 5和α-SMA表达水平低于对照组(P<0.05),补中益气汤组Fibulin 5和α-SMA表达水平高于痔病组(P<0.05).(2)细胞实验结果:①痔病患者原代成纤维细胞增殖和迁移能力均低于对照组(P<0.01),补中益气汤组细胞增殖和迁移能力高于痔病组(P<0.01);②痔病组成纤维细胞中Fibulin 5、LTBP-4、α-SMA、CollagenⅠ蛋白和基因的表达水平低于对照组(P<0.05),补中益气汤组细胞Fibulin 5、LTBP-4、α-SMA、CollagenⅠ蛋白和基因表达水平高于痔病组(P<0.05).③将Fibulin 5敲低后,发现补中益气汤对细胞Fibulin 5、LTBP-4、α-SMA、CollagenⅠ蛋白和基因表达的促进作用被逆转(P<0.05或P<0.01).[结论]补中益气汤可能通过上调Fibulin 5的表达,促进成纤维细胞活化,从而改善痔病病变.

Objective To explore the molecular mechanism by which Buzhong Yiqi Decoction(BYD)ameliorates hemorrhoidal disease.Methods(1)Animal experiment:A hemorrhoidal disease model was established in rats using the topical application of glacial acetic acid.Twenty SD rats were randomly divided into a control group,a model group,and low-,medium-,and high-dose BYD groups,with 4 rats per group.Interventions lasted for 7 consecutive days.Pathological changes in perianal tissues were observed by hematoxylin and eosin(HE)staining.The expression of Fibulin 5 and α-SMA in perianal tissues was detected by immunohistochemistry.(2)Cell experiments:① Primary fibroblasts were extracted from hemorrhoidal tissue samples of patients with Grade Ⅲ prolapsed internal hemorrhoids.Cells were cultured in drug-containing serum media with different concentrations of BYD(10%,20%,30%).The effects of BYD-containing serum on the proliferation and migration of primary fibroblasts were evaluated using the Cell Counting Kit-8(CCK-8)assay and cell scratch wound healing assay,respectively.② Fibroblasts were divided into a control group,a model group,and a BYD group.The protein and gene expression levels of Fibulin 5,LTBP-4,α-SMA,and CollagenⅠ were detected by Western Blot and real-time quantitative polymerase chain reaction(RT-qPCR),respectively.③ Fibulin 5 was knocked down in fibroblasts using siRNA technology.Cells were divided into a control group,a model group,a BYD group,and a BYD+siRNA group.The protein and gene expression levels of Fibulin 5,LTBP-4,α-SMA,and Collagen Ⅰ were detected by Western Blot and RT-qPCR,respectively.Results(1)Results of animal experiments:BYD improved the degree of pathological lesions in the perianal tissues of model rats.The expression levels of Fibulin 5 and α-SMA in the perianal tissues of the model group were lower than those in the control group(P<0.05),while the expression levels in the BYD group were higher than those in the model group(P<0.05).(2)Results of cell experiments:① The proliferation and migration abilities of primary fibroblasts from hemorrhoid patients were lower than those of the control group(P<0.01).The proliferation and migration abilities in the BYD group were higher than those in the model group(P<0.01).② The protein and gene expression levels of Fibulin 5,LTBP-4,α-SMA,and CollagenⅠ in fibroblasts of the model group were lower than those in the control group(P<0.05).The expression levels of these proteins and genes in the BYD group were higher than those in the model group(P<0.05).③ After knocking down Fibulin 5,the promoting effect of BYD on the expression of Fibulin 5,LTBP-4,α-SMA,and CollagenⅠ proteins and genes was reversed(P<0.05 or P<0.01).Conclusion BYD may ameliorate hemorrhoidal lesions by upregulating the expression of Fibulin 5 and promoting fibroblast activation.
