基于异病同治的双石通淋胶囊治疗下尿路刺激症状疾病的临床研究OA
Clinical Study on Shuangshi Tonglin Capsules in Treating Lower Urinary Tract Symptoms Based on the Therapeutic Principle of"Homotherapy for Heteropathy"
[目的]观察双石通淋胶囊治疗具有下尿路刺激症状的慢性前列腺炎及急性下尿路感染的有效性及安全性.[方法]选取2023年12月至2024年7月在广东省第二中医院泌尿外科门诊就诊,具有下尿路刺激症状、中医证型为湿热壅阻型的患者54例.其中,慢性前列腺炎组24例,急性下尿路感染组30例.根据是否使用双石通淋胶囊治疗,分别分为暴露组和非暴露组.其中,慢性前列腺炎暴露组18例,非暴露组6例;急性下尿路感染暴露组20例,非暴露组10例.慢性前列腺炎暴露组口服双石通淋胶囊,非暴露组口服锯叶棕果实提取物(沙芭特),观察时间最长8周,用药疗程不短于4周;急性下尿路感染暴露组口服双石通淋胶囊,非暴露组口服喹诺酮类抗生素治疗,观察时间最长2周.观察慢性前列腺炎、急性下尿路感染暴露组与非暴露组患者治疗前后下尿路刺激症状评分的变化情况,比较慢性前列腺炎、急性下尿路感染暴露组与非暴露组患者的下尿路刺激单项症状(尿频、尿急、尿痛、尿道灼热)消失时间、不同时间点中医证候疗效和不良反应发生率,分析慢性前列腺炎暴露组与非暴露组患者治疗前后美国国立卫生研究院慢性前列腺炎症状指数(NIH-CPSI)评分的变化情况.[结果](1)脱落情况方面,慢性前列腺炎暴露组脱落6例,非暴露组无脱落病例,最终完成临床观察18例(包括暴露组12例和非暴露组6例);急性下尿路感染暴露组脱落3例,非暴露组脱落1例,最终完成临床观察26例(包括暴露组17例和非暴露组9例).(2)中医证候疗效方面,慢性前列腺炎患者治疗2、4、8周后,暴露组的有效率均高于非暴露组,但差异均无统计学意义(P>0.05);急性下尿路感染患者治疗1、2周后,暴露组的有效率均低于非暴露组,但差异均无统计学意义(P>0.05).(3)下尿路刺激症状评分方面,治疗后,慢性前列腺炎、急性下尿路感染暴露组与非暴露组患者的下尿路刺激症状评分均较治疗前降低(P<0.01),但治疗后组间比较,差异均无统计学意义(P>0.05).(4)下尿路刺激单项症状消失时间方面,慢性前列腺炎非暴露组的尿频、尿急消失时间明显短于暴露组(P<0.05),尿痛消失时间略短于暴露组,但差异无统计学意义(P>0.05);暴露组尿道灼热消失时间短于非暴露组,但差异无统计学意义(P>0.05).急性下尿路感染非暴露组的尿频、尿痛消失时间明显短于暴露组(P<0.05);而尿急、尿道灼热组间比较,差异均无统计学意义(P>0.05).(5)不良反应方面,慢性前列腺炎和急性下尿路感染暴露组的不良反应发生率分别为33.33%(4/12)、17.65%(3/17),与非暴露组的16.67%(1/6)和0.00%(0/9)比较,差异均无统计学意义(P>0.05).(6)NIH-CPSI评分方面,治疗2、4、8周后,慢性前列腺炎暴露组与非暴露组患者的NIH-CPSI评分均较治疗前大幅度降低(P<0.01),但治疗后各观察时点组间比较,差异均无统计学意义(P>0.05).[结论]双石通淋胶囊治疗下尿路刺激症状相关疾病的总体疗效与常规西药治疗相当,但在减少抗生素使用和长期症状管理方面显示出独特优势.
Objective To observe the efficacy and safety of Shuangshi Tonglin Capsules in treating chronic prostatitis and acute lower urinary tract infection,both presenting with lower urinary tract symptoms.Methods Fifty-four patients with lower urinary tract symptoms and the traditional Chinese medicine(TCM)syndrome type of dampness-heat obstruction,who visited the Urology Outpatient Clinic of Guangdong Second Hospital of Traditional Chinese Medicine from December 2023 to July 2024 were selected.These patients included 24 cases of chronic prostatitis and 30 cases of acute lower urinary tract infection.Based on whether Shuangshi Tonglin Capsules were used,they were further divided into exposed and non-exposed groups.The chronic prostatitis group comprised 18 exposed and 6 non-exposed cases;the acute lower urinary tract infection group comprised 20 exposed and 10 non-exposed cases.The chronic prostatitis exposed group received oral use of Shuangshi Tonglin Capsules,while the non-exposed group received oral use of Serenoa repens fruit extract(Sabarte).The observation period was up to 8 weeks,with a treatment course not shorter than 4 weeks.The acute lower urinary tract infection exposed group received oral Shuangshi Tonglin Capsules,while the non-exposed group received oral quinolone antibiotics,with an observation period up to 2 weeks.Changes in lower urinary tract irritation symptom scores were observed before and after treatment in both the exposed and non-exposed groups for each disease.The time for disappearance of individual lower urinary tract symptoms(frequent urination,urgent urination,dysuria,urethral burning),TCM syndrome efficacy at different time points,and the incidence of adverse reactions were compared between the exposed and non-exposed groups for each disease.Changes in the National Institutes of Health Chronic Prostatitis Symptom Index(NIH-CPSI)scores were analyzed before and after treatment for the chronic prostatitis exposed and non-exposed groups.Results(1)Regarding study attrition,in the chronic prostatitis group,6 cases dropped out from the exposed group and none from the non-exposed group,resulting in 18 cases completing clinical observation(12 exposed and 6 non-exposed).In the acute lower urinary tract infection group,3 cases dropped out from the exposed group and one from the non-exposed group,resulting in 26 cases completing clinical observation(17 exposed and 9 non-exposed).(2)Regarding TCM syndrome efficacy,for chronic prostatitis patients,the effective rates in the exposed group were higher than those in the non-exposed group at 2,4,and 8 post-treatment weeks,but the differences were not statistically significant(P>0.05).For acute lower urinary tract infection patients,the effective rates in the exposed group were lower than those in the non-exposed group at one and 2 post-treatment week(s),but the differences were not statistically significant(P>0.05).(3)Regarding lower urinary tract irritation symptom scores,after treatment,scores were significantly decreased compared to before treatment in both the exposed and non-exposed groups for chronic prostatitis and acute lower urinary tract infection(P<0.01).However,no statistically significant differences were found between the groups after treatment for either condition(P>0.05).(4)Regarding the disappearance time of individual lower urinary tract irritation symptoms,in the chronic prostatitis group,the disappearance time for frequent urination and urgent urinetion was significantly shorter in the non-exposed group than in the exposed group(P<0.05).The disappearance time for dysuria was slightly shorter in the non-exposed group,but there was no statistically significant difference(P>0.05).The disappearance time for urethral burning was shorter in the exposed group,but there was no statistically significant(P>0.05).In the acute lower urinary tract infection group,the disappearance times for urinary frequency and dysuria were significantly shorter in the non-exposed group(P<0.05).No statistically significant differences were found between groups for urgent urination and urethral burning(P>0.05).(5)Regarding adverse reactions,the incidence in the chronic prostatitis and acute lower urinary tract infection exposed groups was 33.33%(4/12)and 17.65%(3/17),respectively.Compared to the non-exposed groups[16.67%(1/6)and 0.00%(0/9)],the differences were not statistically significant(P>0.05).(6)Regarding NIH-CPSI scores,after 2,4,and 8 weeks of treatment,the scores were significantly decreased compared to before treatment in both the exposed and non-exposed chronic prostatitis groups(P<0.01).However,no statistically significant differences were found between the groups at any post-treatment observation time point(P>0.05).Conclusion The overall efficacy of Shuangshi Tonglin Capsules in treating diseases with lower urinary tract symptoms is comparable to conventional western medicine treatments.However,it presents unique advantages in reducing antibiotic use and for long-term symptom management.
