美国成年人硒摄入量与前庭平衡功能的关联研究OA
Association between selenium intake and vestibular balance function in U.S.adults
目的 了解硒摄入与前庭平衡功能的关系,以期为预防前庭相关疾病提供新的方法.方法 数据来源于2021-2023年美国国家健康与营养调查(NHANES)的3 334名美国成年人.调查员通过2次24 h膳食回顾法评估参与者的膳食硒和硒补充剂的总摄入量,并采用改良Romberg测试结果作为评估前庭平衡功能的客观指标.应用加权logisitc回归模型分析硒摄入量与改良Romberg测试阳性的关联,并采用限制性立方样条(RCS)模型分析二者剂量-反应关系.结果 研究样本经加权后代表188 916 729名美国成年人,人群总体改良Romberg测试阳性率为51.5%,其中高硒摄入量组的糖尿病患病率最高(11.3%).多因素logisitc回归分析显示,在调整混杂因素后,与低硒摄入量组相比,中等硒摄入量组的改良Rom-berg测试阳性风险显著下降(OR=0.74,95%CI:0.58~0.94,P=0.013).RCS分析表明,硒摄入量与改良Romberg测试阳性率呈显著的"U型"非线性剂量-反应关系(非线性P值为0.021),当硒摄入量为100~250 μg/d时,摄入硒与改良Romberg测试阳性呈负相关.结论 美国成年人的硒摄入量与改良Romberg测试阳性率呈"U型"关系,表明适宜的硒摄入才能在维持前庭平衡功能方面获得最佳收益.此外,高硒摄入与糖尿病状态相关,这可能影响硒摄入量与前庭平衡功能之间的关联.因此,在评估与制定临床营养策略时,应综合考虑个体的硒营养状况与糖尿病风险,以推动针对前庭功能障碍的个性化预防与管理策略的制定.
Objective To investigate the association between selenium intake and vestibular balance function,so as to provide a new approach for the prevention of vestibular-related diseases.Methods Data were obtained from 3,334 U.S.adults par-ticipating in the National Health and Nutrition Examination Survey(NHANES)from 2021 to 2023.Interviewers assessed total dietary and supplemental selenium intake using two 24-hour dietary recalls,and the modified Romberg test result was used as an objective indicator of vestibular balance function.Weighted logistic regression models were applied to analyze the association between selenium intake and positive modified Romberg test results,and restricted cubic spline(RCS)models were used to examine the dose-response relationship.Results After weighting,the study sample represented 188,916,729 U.S.adults.The overall prevalence of positive modified Romberg test results was 51.5%.The prevalence of diabetes was the highest in the high selenium intake group(11.3%).Multivariate logistic regression analysis showed that,after adjusting for confounding factors,the risk of a positive modified Romberg test result was significantly lower in the medium selenium intake group compared with the low selenium intake group(OR=0.74,95%CI:0.58-0.94,P=0.013).RCS analysis showed a significant U-shaped nonlinear dose-response relationship between selenium intake and the rate of positive modified Romberg test results(P for nonlinearity=0.021).When selenium intake was 100-250 μg/d,selenium intake was nega-tively associated with positive modified Romberg test results.Conclusion The relationship between selenium intake and the rate of positive modified Romberg test results in U.S.adults is U-shaped,indicating that an appropriate level of selenium intake is necessary to achieve optimal benefits for maintaining vestibular balance function.Furthermore,high selenium in-take is associated with diabetes status,which may affect the association between selenium intake and vestibular balance func-tion.Therefore,when assessing and formulating clinical nutrition strategies,an individual's selenium nutritional status and diabetes risk should be comprehensively considered to promote the development of personalized prevention and management strategies for vestibular dysfunction.
