"补肝肾、强筋骨"类中药及活性成分在防治退行性骨病中的研究进展OA
Research progress of"tonifying liver and kidney,strengthening muscles and bones"traditional Chi-nese medicine in the prevention and treatment of degenerative bone diseases
退行性骨病属中医"骨痹"范畴,肝肾亏虚、筋骨失养为其核心病机,基于中医"肝肾同源、主骨生髓"理论,补肝肾、强筋骨类中药及其活性成分是防治骨质疏松、骨关节炎等退行性骨病的重要方向.本文系统梳理了补肝肾、强筋骨类中药的性味归经、典籍记载与活性成分现代药理作用,深入阐释了该类中药及活性成分防治退行性骨病的核心药效与分子机制.该类中药及活性成分可通过激活Wnt/β-连环蛋白(Wnt/β-catenin)、骨形态发生蛋白/Smad蛋白(bone morphogenetic protein/mothers against decapentaplegic homolog,BMP/Smad)等通路促进成骨细胞增殖分化,调控核因子κB受体活化因子配体/核因子κB受体活化因子/骨保护素(receptor activator of nuclear factor-κB ligand/receptor activator of nuclear factor-κB/osteoprotegerin,RANKL/RANK/OPG)系统及核因子κB(nuclear factor-κB,NF-κB)等通路抑制破骨细胞生成与骨吸收,还能借助NF-κB、丝裂原活化蛋白激酶(mitogen-activated protein kinase,MAPK)、磷脂酰肌醇3-激酶/蛋白激酶B/哺乳动物雷帕霉素靶蛋白(phosphatidylinositol 3-kinase/protein kinase B/mammalian target of rapamycin,PI3K/Akt/mTOR)等通路发挥软骨保护与抗炎作用,从促成骨、抑破骨、护软骨多维度调节骨代谢平衡,体现出多靶点、多通路的整体调节特点.为进一步明确补肝肾、强筋骨类中药的药理基础、推动该类中药及其活性成分的现代化应用与新药研发提供了理论参考与研究思路.
Degenerative bone diseases fall into the category of"Bi"syndrome of bone in traditional Chinese medi-cine(TCM),with liver-kidney deficiency and malnourishment of tendons and bones as the core pathogenesis.Based on the TCM theory that"the liver and kidney are homologous,and the kidney governs bones and generates marrow",Chinese herbal medicines for tonifying the liver and kidney,and strengthening tendons and bones and their active ingredients represent an important strategy for the prevention and treatment of degenerative bone dis-eases such as osteoporosis and osteoarthritis.This paper systematically reviews the nature,taste,meridian tropism,classical records,and modern pharmacological effects of these herbs and their active ingredients,and elaborates on their core efficacy and molecular mechanisms against degenerative bone diseases.These herbs and active ingredi-ents can promote the proliferation and differentiation of osteoblasts by activating pathways including Wnt/β-catenin and bone morphogenetic protein/mothers against decapentaplegic homolog(BMP/Smad),inhibit osteo-clastogenesis and bone resorption by regulating the receptor activator of nuclear factor-κB ligand/receptor activa-tor of nuclear factor-κB/osteoprotegerin(RANKL/RANK/OPG)system and the nuclear factor-κB(NF-κB)path-way,and exert chondroprotective and anti-inflammatory effects by the NF-κB,mitogen-activated protein kinase(MAPK),and phosphatidylinositol 3-kinase/protein kinase B/mammalian target of rapamycin(PI3K/Akt/mTOR)pathways.They regulate bone metabolic homeostasis by multi-dimensional effects including promoting osteogen-esis,inhibiting bone resorption,and protecting cartilage,which demonstrates the characteristics of multi-target and multi-pathway holistic regulation.This review provides a theoretical basis and research insights for further clarify-ing the pharmacological basis of these herbs,and for promoting their modern application and new drug develop-ment.
