养血固肾汤对髋部骨折术后肾虚型骨质疏松患者骨代谢生化指标的影响OA
Effects of the Blood-nourishing and Kidney-tonifying Decoction on Bone Metabolism Biochemical Markers in Patients with Kidney-deficiency-type Osteoporosis Following Hip Fracture Surgery
目的:探讨养血固肾汤对髋部骨折术后肾虚型骨质疏松患者康复效果及骨代谢生化指标的影响.方法:采用前瞻性研究方法,选取髋部骨折术后肾虚型骨质疏松患者60例,按随机数字表法分为两组,每组30例.对照组术后给予常规西医治疗,观察组在对照组基础上加用养血固肾汤治疗,两组均持续治疗3个月.观察两组临床疗效、不良反应发生率及治疗前后症状评分、骨密度及骨代谢生化指标的变化情况.结果:观察组总有效率[96.67%(29/30)]高于对照组[76.67%(23/30)](P<0.05).两组术后不良反应发生率比较,差异无统计学意义(P>0.05).与治疗前比较,治疗后两组腰膝疲软、畏寒肢冷、头晕耳鸣等症状评分均降低(P<0.05),髋部骨密度、股骨大粗隆骨密度均增加(P<0.05),血清骨钙素、碱性磷酸酶、总Ⅰ型前胶原氨基端延长肽、胰岛素样生长因子Ⅰ水平均上升(P<0.05),全段甲状旁腺素、Ⅰ型胶原羧基末端肽水平均下降(P<0.05);且观察组治疗后各指标改善程度优于对照组(P<0.05).结论:髋部骨折术后肾虚型骨质疏松患者在常规西医治疗基础上,加用养血固肾汤口服治疗,可显著改善骨密度及骨代谢生化指标,提高临床疗效,促进骨折术后康复.
Objective:To explore the effects of the blood-nourishing and kidney-tonifying decoction on the recovery and bone metabolism biochemical markers in patients with kidney-deficiency-type osteoporosis after hip fracture surgery.Methods:A prospective study was conducted,in which sixty patients with kidney-deficiency-type osteoporosis after hip fracture surgery were selected and divided into two groups using a random number table,with thirty cases in each group.The control group received conventional Western medical treatment after surgery,while the observation group was additionally treated with the blood-nourishing and kidney-tonifying Decoction based on the treatment regimen of the control group.Both groups were treated continuously for three months.To observe clinical effects,the incidence of adverse reactions,as well as changes in symptom scores,bone mineral density,and bone metabolism biochemical markers before and after treatment in both groups.Results:The total effective rate in the observation group[96.67%(29/30)]was higher than that in the control group[76.67%(23/30)](P<0.05).There was no statistically significant difference in the incidence of postoperative adverse reactions between the two groups(P>0.05).Compared with before treatment,both groups showed reductions in symptom scores for soreness and weakness of the waist and knees,aversion to cold and cold limbs,and dizziness and tinnitus after treatment(P<0.05),increases in hip bone mineral density and femoral greater trochanter bone mineral density(P<0.05),elevations in serum levels of osteocalcin,alkaline phosphatase,total procollagen type I N-terminal propeptide,and insulin-like growth factor I(P<0.05),and decreases in serum levels of intact parathyroid hormone and C-terminal telopeptide of type I collagen(P<0.05).Moreover,the improvements in these indicators were more significant in the observation group than in the control group after treatment(P<0.05).Conclusion:For patients with kidney-deficiency-type osteoporosis following hip fracture surgery,the addition of oral administration of the blood-nourishing and kidney-tonifying decoction to conventional Western medical treatment can significantly improve bone mineral density and bone metabolism biochemical markers,enhance clinical efficacy,and promote postoperative recovery from fractures.
