MSCT联合超声在TI-RADS 4类甲状腺结节定性诊断中的价值OA
Value of MSCT Combined with Ultrasound in Qualitative Diagnosis of TI-RADS 4 Thyroid Nodules
目的:分析多层螺旋电子计算机断层扫描(multi-slice spiral computed tomography,MSCT)联合超声在甲状腺影像报告与数据系统(thyroid imaging reporting and data system,TI-RADS)4类甲状腺结节定性诊断中的价值.方法:选取2021年7月至2022年7月在本院诊治的121例(121个结节)TI-RADS 4 类甲状腺结节患者为研究对象,所有患者均接受 MSCT、超声检查,比较患者 MSCT、超声检查结果,分析其诊断TI-RADS 4类甲状腺结节良恶性价值.结果:以病理学结果为金标准,经病理学诊断确定121例TI-RADS 4类甲状腺结节患者中,87例恶性结节,34例良性结节;MSCT检出41例良性结节,80例恶性结节;与病理学诊断对照,MSCT鉴别TI-RADS 4类甲状腺结节良恶性敏感度为85.06%,特异度为82.35%;超声检出44例良性结节,77例恶性结节;与病理学诊断对照,超声鉴别 TI-RADS 4 类甲状腺结节良恶性敏感度为 79.31%,特异度为76.47%;MSCT、超声及其联合诊断TI-RADS 4类甲状腺结节良恶性的敏感度、准确性、阴性预测值比较差异有统计学意义(P<0.05),特异度、阳性预测值比较差异无统计学意义(P>0.05);联合诊断敏感度、特异度、准确性、阳性预测值、阴性预测值均高于超声诊断,敏感度、准确性高于MSCT(P<0.05).结论:MSCT联合超声诊断能提高TI-RADS 4类甲状腺结节良恶性诊断的敏感度、准确性.
Objective:To analyze the value of multi-slice spiral computed tomography(MSCT)combined with ultrasound in qualitative diagnosis of Thyroid Imaging Reporting and Data System(TI-RADS)4 thyroid nodules.Methods:A total of 121 patients with TI-RADS 4 thyroid nodules(121 in total)who were diagnosed and treated in the hospital from July 2021 to July 2022 were selected as research subjects.All patients underwent MSCT and ultrasound examinations,and the examination results were comparatively analyzed.Their value in qualitative diagnosis of TI-RADS 4 thyroid nodules was evaluated.Results:Pathological results(the gold standard)showed that among 121 patients with TI-RADS 4 thyroid nodules,there were 87 cases with malignant nodules and 34 cases with benign nodules.MSCT found 41 cases with benign nodules and 80 cases with malignant nodules.Compared with pathological diagnosis,the sensitivity and specificity of MSCT for distinguishing benign and malignant TI-RADS 4 thyroid nodules were 85.06%and 82.35%.44 cases with benign nodules and 77 cases with malignant nodules were found by ultrasound examinations.Compared with pathological diagnosis,the sensitivity and specificity of ultrasound for distinguishing benign and malignant TI-RADS 4 thyroid nodules were 79.31%and 76.47%.The sensitivity,accuracy,and negative predictive values of MSCT,ultrasound,and combination of the two for distinguishing benign and malignant TI-RADS 4 thyroid nodules showed statistically significant differences(P<0.05).However,the specificity and positive predictive values showed no statistically significant difference(P>0.05).The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of combined diagnosis were higher than ultrasound diagnosis.The sensitivity and accuracy were higher than MSCT(P<0.05).Conclusion:MSCT combined with ultrasound can improve the diagnostic sensitivity and accuracy for benign and malignant TI-RADS 4 thyroid nodules.
