神经鞘瘤与神经纤维瘤超声影像特征对比及诊断一致性分析OA
Comparison of Ultrasound Imaging Features between Schwannoma and Neurofibroma and Analysis of Diagnostic Consistency
目的:探究神经鞘瘤与神经纤维瘤超声影像特征对比及诊断一致性分析.方法:本研究采用连续性纳入方式,选取我院于2020年1月至2024年11月期间的神经鞘瘤患者39例作为神经鞘瘤组及41例神经纤维瘤患者作为神经纤维瘤组.利用超声检查评估两组患者肿瘤特征,并结合临床病理特征进行一致性分析.利用Kappa分析超声特点、临床病理特征的一致性;并通过ROC曲线分析联合诊断的价值.结果:与神经纤维瘤组相比,神经鞘瘤组在最小径、位置(深在)、最大径、边界不清晰、Ki-67、形态不规则、囊实性、梭形、栅栏状、包膜、囊性成分(无)、血流分级3级方面存在显著差异(P<0.05).Kappa分析结果显示,超声特点诊断与临床病理诊断一致性较强(Kappa=0.791).ROC 曲线结果显示,临床病理特征联合超声特点诊断的 AUC 值(0.804)、敏感度(94.87%)及准确度(80.00%)均显著高于单一诊断方法,说明联合诊断具备更高准确度及敏感度.结论:本研究通过对比神经鞘瘤与神经纤维瘤的超声特点及临床病理特征,发现超声特点与临床病理诊断之间具有较强的一致性(Kappa=0.791),表明超声检查在神经鞘瘤的诊断中具有较高的可靠性.此外,超声特点与临床病理特征联合诊断的AUC值、敏感度及准确度均高于单一诊断方法,进一步证实了两者联合应用在神经鞘瘤诊断中的重要价值.因此,超声检查可作为神经鞘瘤临床诊断的重要辅助手段,与临床病理特征相结合可显著提高诊断的准确性和敏感度.
Objective:To explore the contrast and consistency analysis of ultrasound imaging features between schwannoma and neurofibroma.Methods:This study adopted a continuous inclusion method and selected 39 patients with schwannoma from our hospital between January 2020 and November 2024 as the schwannoma group and 41 patients with neurofibroma as the neurofibroma group.Evaluated the tumor characteristics of two groups of patients using ultrasound examination,and consistency analysis was performed based on clinicopathologic features.The Kappa coefficient was used to evaluate the consistency of ultrasound imaging features and clinicopathologic features;and analyze the value of joint diagnosis through ROC curve analysis.Results:Compared with the neurofibroma group,there were significant differences in the smallest diameter,location(depth),maximum diameter,unclear boundary,Ki-67,irregular shape,cystic-solid nature,spindle shape,palisade,envelope,cystic component(none)and blood flow grade 3 in the schwannoma group(P<0.05).The Kappa analysis results showed a strong consistency between ultrasound characteristic diagnosis and clinical pathological diagnosis(Kappa=0.791).The ROC curve results showed that the AUC value(0.804),sensitivity(94.87%),and accuracy(80.00%)of clinicopathologic features combined with ultrasound imaging features were significantly higher than those of a single diagnostic method,indicating that combined diagnosis had higher accuracy and sensitivity.Conclusion:This study compared the ultrasound imaging features and clinical pathological features of schwannoma and neurofibroma,and found a strong consistency between ultrasound imaging features and clinical pathological diagnosis(Kappa=0.791),indicating that ultrasound examination has high reliability in the diagnosis of schwannoma.In addition,the AUC value,sensitivity,and accuracy of the combined diagnosis of ultrasound imaging features and clinicopathologic features were higher than those of a single diagnostic method,further confirming the important value of the combined application of the two in the diagnosis of schwannoma.Therefore,ultrasound examination can serve as an important auxiliary tool for clinical diagnosis of schwannoma,and combining it with clinicopathologic features can significantly improve the accuracy and sensitivity of diagnosis.
