增强CT影像组学与胃肠道间质瘤病理参数的相关性及对预后的预测价值OA
Correlation between Enhanced CT Imaging Radiomics and Pathological Parameters of Gastrointestinal Stromal Tumors and its Predictive Value for Prognosis
目的:本研究旨在探究增强CT影像组学特征与胃肠道间质瘤(GIST)病理参数(DOG1、CD117、CD34、Ki-67指数)的相关性,以及其对GIST患者预后的预测价值.方法:选取2016年1月至2024年3月在河北北方学院附属第一医院就诊的220例GIST患者,并根据随访期间影像学或临床证据确认的肿瘤进展(参照 RECIST 1.1 标准)分为未进展组(n=150)和进展组(n=70).所有患者均接受腹部增强 CT 静脉期扫描,借助影像组学方法,系统提取肿瘤的纹理、形态及直方图特征参数.收集病理参数(DOG1、CD117、CD34表达情况、Ki-67指数)及临床资料,运用双变量Pearson相关系数分析影像组学特征与病理参数的相关性,通过Cox比例风险回归模型(调整年龄、性别、肿瘤大小等混杂变量,且经多重共线性检验,VIF<5)筛选独立预后因素,并利用ROC曲线评估影像组学特征的预后预测效能.结果:影像组学特征中,纹理熵与 CD117 表达呈正相关(r=0.224,P<0.01),与 DOG1 强度(r=0.231,P<0.01)、CD34 表达(r=0.208,P<0.01)呈正相关;灰度方差与 CD117 表达呈负相关(r=-0.142,P<0.05),与 DOG1 强度(r=0.159,P<0.05)、CD34 表达(r=0.172,P<0.05)呈正相关;球形度与 CD117 表达呈负相关(r=-0.262,P<0.01),与Ki-67 指数(r=0.187,P<0.01)呈正相关(P<0.05).Cox多因素分析显示,纹理熵(HR=4.374,95%CI:2.204~8.682,P<0.001)、灰度方差(HR=1.024,95%CI:1.013~1.036,P<0.001)、球形度(HR=0.034,95%CI:0.004~0.268,P<0.001)、Ki-67 指数(HR=2.655,95%CI:1.408~5.003,P=0.003)是GIST患者的独立预后因素.ROC曲线分析显示,纹理熵(AUC=0.705,95%CI:0.627~0.782)、灰度方差(AUC=0.733,95%CI:0.654~0.813)、球形度(AUC=0.704,95%CI:0.630~0.778)、Ki-67指数(AUC=0.703,95%CI:0.623~0.784)对 GIST 患者预后不良均有一定预测价值(均P<0.001).结论:增强CT影像组学特征与GIST病理参数密切相关,可能通过反映肿瘤异质性及血管生成等机制关联病理参数,对患者预后具有较高预测价值,可辅助临床评估病情及制定个体化治疗策略.本研究为单中心回顾性设计,未进行外部验证,未来需纳入多中心数据以验证模型普适性.
Objective:This study aimed to investigate the correlation between contrast-enhanced computed tomography(CT)radiomics features and pathological parameters(DOG1,CD117,CD34,Ki-67 index)in gastrointestinal stromal tumors(GIST),and to evaluate their predictive value for patient prognosis.Methods:A total of 220 GIST patients treated at the First Affiliated Hospital of Hebei North University between January 2016 and March 2024 were enrolled.Based on their status during follow-up,they were divided into a non-progression group(n=150)and a progression group(n=70).All patients underwent abdominal contrast-enhanced CT.Radiomics methods were employed to systematically extract texture,morphological,and histogram-based characteristic parameters of the tumors.Pathological parameters(DOG1,CD117,and CD34 expression status,Ki-67 index)and clinical data were collected.The bivariate Pearson correlation coefficient was used to analyze the correlation between radiomics features and pathological parameters.The Cox proportional hazards regression model was applied to screen for independent prognostic factors.The receiver operating characteristic(ROC)curve was utilized to assess the prognostic predictive efficacy of the radiomics features.Results:Among the radiomics features,texture entropy was positively correlated with CD117 expression(r=0.224,P<0.01),and positively correlated with DOG1 expression intensity(r=0.231,P<0.01)and CD34 expression(r=0.208,P<0.01).Gray-level variance was negatively correlated with CD117 expression(r=-0.142,P<0.05),while positively correlated with DOG1 expression intensity(r=0.159,P<0.05)and CD34 expression(r=0.172,P<0.05).Sphericity was negatively correlated with CD117 expression(r=-0.262,P<0.01),and and positively correlated with Ki-67 index(r=0.187,P<0.01)(all P<0.05).Multivariate Cox regression analysis showed that texture entropy(HR=4.374 95%CI:2.204~8.682,P<0.001),gray-level variance(HR=1.024,95%CI:1.013~1.036,P<0.001),sphericity(HR=0.034,95%CI:0.004~0.268,P<0.001)and Ki-67 index(HR=2.655,95%CI:1.408~5.003,P=0.003)were independent prognostic factors for GIST patients.ROC curve analysis demonstrated that texture entropy(AUC=0.705,95%CI:0.627~0.782),gray-level variance(AUC=0.733,95%CI:0.654~0.813),sphericity(AUC=0.704,95%CI:0.630~0.778)and Ki-67 index(AUC=0.703,95%CI:0.623~0.784)all had certain predictive value for poor prognosis in GIST patients(all P<0.001).Conclusion:Contrast-enhanced CT radiomics features are closely associated with pathological parameters in GIST and demonstrate high predictive value for patient prognosis.They can assist in clinical assessment of disease status and the formulation of personalized treatment strategies.
