益生菌对局部晚期宫颈癌放射性损伤的影响OA
The effect of probiotics on radiation damage in locally advanced cervical cancer
目的 探讨使用益生菌对局部晚期宫颈癌患者急性放射性损伤的影响.方法 回顾性分析2019年1月至2024年4月在重庆医科大学附属第三医院(方大医院)行放化疗初治并经组织病理学证实为局部晚期宫颈癌的148例患者临床资料.根据是否使用益生菌分为益生菌组(n=84)和对照组(n=64),分析使用益生菌对急性放射性直肠炎、膀胱炎及胃肠炎发生率的影响.结果 益生菌组外照射剂量>50.4 Gy和接受阴道近距离治疗比例高于对照组,差异有统计学意义(P<0.05).使用益生菌较未使用者的急性放射性直肠炎、膀胱炎发生率更低,化疗、外照射剂量>50.4 Gy患者的急性放射性胃肠炎发生率分别较未化疗、外照射剂量≤50.4 Gy更大,差异有统计学意义(P<0.05).多因素logistic回归分析结果显示,使用益生菌较未使用者的急性放射性直肠炎、膀胱炎发生率降低82.8%(OR=0.172,95%CI:0.080~0.371)、51.5%(OR=0.485,95%CI:0.244~0.965),外照射剂量>50.4 Gy、行阴道近距离治疗患者急性放射性胃肠炎发生率分别是≤50.4 Gy、未行阴道近距离治疗的4.547倍(OR=4.547,95%CI:1.796~11.514)、5.502倍(OR=5.502,95%CI:0.690~43.888).结论 使用益生菌可降低局部晚期宫颈癌放射性直肠炎、膀胱炎发生率.
Objective To explore the effect of using probiotics on the acute radiation injury in patients with locally advanced cervical cancer.Methods A retrospective analysis was conducted on the clinical data of 148 patients who were initially treated with radiotherapy and chemotherapy at the Third Affiliated Hospital of Chongqing Medical University from January 2019 to April 2024 and were confirmed to have locally advanced cervical cancer by histopathology.The patients were divided into the probiotic group(n=84)and the control group(n=64)based on whether probiotics were used.The effects of probiotics on the incidence of acute radi-ation proctitis,cystitis and gastroenteritis were analyzed.Results The proportion of patients with an external irradiation dose>50.4 Gy and receiving vaginal brachytherapy was higher in the probiotic group than in the control group,with statistically significant differences(P<0.05).The incidence of acute radiation proctitis and cystitis was lower in the probiotic group than in the control group.The incidence of acute radiation gastro-enteritis was greater in patients undergoing chemotherapy than in those not,and in those with an external irra-diation dose>50.4 Gy than in those with≤50.4 Gy,with statistically significant differences(P<0.05).The results of multivariate logistic regression analysis showed that the probiotic users had 82.8%(OR=0.172,95%CI:0.080-0.371)and 51.5%(OR=0.485,95%CI:0.244-0.965)reduced incidence of radiation proc-titis and cystitis compared to non-users.The incidence of acute radiation gastroenteritis in patients with an ex-ternal irradiation dose>50.4 Gy and those receiving vaginal brachytherapy was 4.547 times(OR=4.547,95%CI:1.796-11.514)and 5.502 times(OR=5.502,95%CI:0.690-43.888)that of patients with≤50.4 Gy and those without vaginal brachytherapy,respectively.Conclusion The use of probiotics can reduce the in-cidence of radiation-induced proctitis and cystitis in patients with locally advanced cervical cancer.
